I've viewed a few houses now where there's no sink in the downstairs loo, which I think is pretty Envy

Maybe something like this would work? Cold water and drainage probably pretty easy but not sure about hot water.

Looks like an outside wall - they could plumb from behind.

My bil has a downstairs loo similar to yours and he fitted a mini corner sink with slim unit. I don't think he ripped out the existing tiles.

We added a little corner sink. We didn't have the budget to retile at the time, so the pipes for the sink just run along clipped to the wall. We'll get them boxed in or sunk into the wall one day when we have the room re-done.

I wouldn't run water pipes outside and plumb from the back, as the pipes will freeze in cold weather. The only pipes running externally are usually drainage, or gas pipes.

some are very small

In mine the pipes are concealed behind rectangular clip-on white plastic duct. You can paint it if you feel the need.
