Bathroom dos and don'ts?

We need to sort out our downstairs toilet. At the moment, I'm not quite sure about orientation and options for fittings. It's a square room, and we could either have toilet and sink opposite each (as they are now), but only problem is there isn't a lot of space to open the cupboard under the sink and see what's in there. Or we could have them side by side.

In terms of fittings, all options are open - close coupled, concealed cistern or wall hung toilet and either pedestal or vanity for the sink. Any thoughts in terms of pros and cons with cleaning or things that can go wrong? We do have room for some shelving or a cupboard in one corner of the room if we don't go for a vanity sink. This would be easier to access if the toilet and sink remain opposite each other, as they are now.

My advice is to have a piece of stone (granite etc) cut to sit on the surface over the boxed cistern/behind the sink. That way you can lift it off for access if something goes wrong and it’s easy to keep clean.
Make sure you have enough room to angle a mop under the vanity for cleaning without the other end bashing the opposite wall
