Who do I need - designer/architect/someone else?

We've just bought a house and the downstairs layout is somewhat strange. It's very open plan, with a walk in pantry, a nice size laundry, a couple of living rooms and a bathroom. The kitchen and all the flooring needs replacing. This is our long-term home, we're not expecting to move again in the next 20 or so years.

What we need is to work with someone who can help us choose the layout for the whole of the downstairs - who will help us plan removing of walls, moving round of fittings, choosing the function for each room and so on. Ultimately, we'd expect this project to go on for years as we get the money together, so we'd want to say: 2019 we'll replace the kitchen and knock out this wall to extend the dining area, 2020 we'll remove this bathroom and refloor that room to make a cosier living room etc etc.

So my question is: who do we need to look for? Is this an architect job? Is it an interior designer? Someone else I've never heard of?

We're not in the UK if that matters, so I don't need actual recommendations - just an idea of where to start looking!

Architect I think. One that specialises in domestic renovations.

My friend is an interior architect who does exactly this but can also advise of extensions... I mean she could build you a whole house if you want. She is also an interior designer which helps enormously with thinking about finishes and so on. Worth considering I think.
