I am looking at taking DD out of her primary school for various reasons & moving her into the independent sector. I don't want to tell her school until she has a place, but what do I do then? Arrange a meeting with the head teacher? Fill in a form/ Write a letter? Any advice welcomed. Cheers.

We did this... moved from one state to another one at the beginning of year 3. When you're ready to make the move, let the school secretary know. I never even dealt with the head. It's not a big drama. They'll send over the records to the new school. Just make sure the secretary at the new school has the contact details for the previous school so he/she can liaise and get further records or missing details. There's very little you need to do other than support your DD. Change is always a bit unsettling, but not for long. They make new friends.

When we did this with dd2 we sent in a letter stating that she would not be returning after easter holiday vut would move to x school.

Acting head was not happy about it but likely more because she feared a complaint than due to the move. The school made a card for DD2 And wished her well.

It was the right decision to make dd2 thrived in the private school we picked.

I think that they always speak to the existing school out of courtesy before offering a place - annoying I know!

Well, in state schools, the LA do all the offering of places and liaising (and Heads cannot refuse to accept as long as there's room), but the Head Teachers will almost certainly speak on the phone as well. The new school will contact the old one for written records to be transferred - parents do not need to involve themselves with that.
With a private school, however, they have discretion over who they offer places to, so I would expect they would ask for some background from the previous school first. That's not to say they'd believe everything they were told, however.

if the old school is a state school, is it obliged to hand over any records of levels etc to the new school, if the new school is private?

I moved my son between state schools, & we just wrote a letter. It all happened very fast, iirc less than a week between letter & leaving date. Didn't cause any issues.
