do I ask to see a accident form?

dd had a head injury a few weeks ago which needed a paramedic. I asked the school if there was any forms I'd need to sign. they said yes but because a paramedic was called and medical assistance was needed it'll take a while for a report to come back.

do I ask to see the report or accident form when dd goes back on the 7th?

Sorry, I don't know about accident report forms. I would just want to know how the injury had happened and who assessed her and how quickly the paramedic was called. Hope she is OK now

Here is the Health & Safety Executive (HSE) procedure for accidents in schools: www.hse.gov.uk/pubns/edis1.pdf.

As a parent you are entitled to understand what happened and what actions the school took to deal with the situation. You are also entitled to enquire if this was an avoidable accident and whether the school are now reviewing the incident in order to mitigate against any future accidents of a similar nature.

It may be given the gravity of the incident the priority was ringing a paramedic rather than filling in the accident form - for which the school should be forgiven. Perhaps I'm reading too much into your post - but what I think you need to understand was whether there was a procedure and this incident was handled well.

No she wasn't. A paramedic was called to.the school because she was concious and hadn't passed out at all. What I'm also worried about is the fact I know another child was hurt and I know that it was due to a little boy who was above dd and her friends around 8ft above her and he picked up the object and dropped it on dds head. Object didn't smash, did bounce off dds head, then her shoulder then hit the child next to her on the leg. I'm not interested in what happened to the child who did it because I also know there was an adult present with him who didn't prevent it or help dd. It was another child who went to get help for dd.

How an accident/incident is reported/recorded depends on circumstances.

An accident involving hospitalisation is automatically reported under the link PSBD posted

Other accidents/incidents would be recorded in the school accident book while others would be reported to the LEA and each LEA has their own regulations.

It's worth looking at your LEA website for their guidance
