in 25 years of teaching, and then my own schooling, toilets were not accessible during lessons unless you had a medical reason, so that hasnt changed.
But we know many children are growing up in poverty and where there is child poverty there will be god-awful diets. Lack of fresh fruit and veg. More carbs lacking nutrition. pizza and chips again anyone?

Poor children have a poor diet. Poor diet harms gut health.
In child poverty there is also a lack of exercise, clubs, activities.
‘Free access to toilets’ screams laziness. It does nothing to address the actual issues with poor children, poor parenting, societies issues. And who pays for the damage?
