Should I take my DD out of preschool/nursery after only 3 days?

My dd started in the nursery class of the large local primary school this week. The setting is huge and I was worried about her finding the loo, eating her lunch etc.
At the end of day one I picked her up and she was wet.
On the morning of day 2 I spoke to the teacher who said they'd make sure she went to the loo during the day.
At the end of day 2 I picked her up and she was wet and clearly had been for hours- because her pants had started to dry and she was also slightly solied.
On the morning of day 3 I spoke to the teacher again and she assured me that they were able to take her to the loo and help her change if necessary.
At the end of day 3 she had to go to after school club for 1 hour. When I arrived to collect her she was wet and soiled. She had no idea where the loo was and i was led a long way to one to clean her up.
I'm distraught!
She is 4 and although not the perfect potty training student, I've never collected her from childcare in such a state and she's not soiled herself for a very long time.
I don't believe that this school can care for her at all.
My dd said she couldn't tell anyone she needed the loo, she couldn't remember the names of the staff, she just seemed confused about it and didn't want to talk about it. She is normally very confident and outgoing and I've never had to worry about her before so this is completely new.
The school have an ofsted outstanding rating which is why I sent her there, but the care doesn't seem outstanding to me.
Is this normal? Should I try to move her to another school?

How upsetting for you- and her. As you have voiced it several times now I think it's time to take it further. I would ask to speak to the head of Early Years. Also she must have a key person so even tho it is large and busy the key person should be keeping a special eye on her new little ones.

Is this a state school? A full day plus after school club this early in the year is too much. Is this the 30 hours thing kicking in? Previously school nurseries were always 3-hour sessions.
