Hello all,

Sorry in advance for the ridiculously long post. I just wanted to give context to my question.

This is my first time posting, but just looking for some stories of how things turned out from other people in a similar position to me. I know everyone's pregnancy's are different, but I don't personally know anyone who has has anything similar. So, I'll try to be brief as possible with my story.

I'm 37 and this is my first child. About 10 years ago I had LLETZ surgery on my cervix and have had a couple biopsies since. I was told at the time that if I ever decided to have a baby, I may need a stitch, but it would be determined at the time.
Anyways fast forward and I was referred to the pre-term birth clinic when I found out I was expecting and was booked in for when I was 19+1 weeks pregnant. My pregnancy went without problem (other than bad morning sickness) until I was 15+6 and I experienced brown discharge. I was given anti-d at this time as I'm rhesus negative and they hadn't yet had babies blood type back.

Then at 19 weeks, just before my first appointment at the pre-term birth clinic I had another loss of pink discharge. When I went to the appointment I was told my cervix was 2.8cm and I wouldn't need a stitch, but was put on pogesterone 200mg. I was also sent up to triage for an actim partus test, which came back negative. By this point I found out baby also had a negative blood type so didn't need anti-d again.

At 20+3 weeks I had a proper bleed, which was like a gush and then stopped. I was checked over by triage and because the bleeding had stopped and there was no active signs of labour I was sent home and told to rest.

At my next pre-term birth clinic appointment (21+1 weeks) by cervix was measuring 2.6cm, so they upped my pogesterone to 400mg to see if that helped. Then on the Friday (21+4 weeks) I had another gush of blood and this time was admitted to hospital and sent for another scan and cervix scan and all looked fine and my cervix was showing to be 3.1cm. I was discharged and advised to take some time off work and rest. Which I did and then at my final preterm birth clinic appointment (23+1 weeks) everything was looking fine and my cervix was still measuring above 3cm, so I was left on pogesterone 400mg and was to be reviewed at 32 weeks.

All went fine and I got back to work. Then at 29+2 weeks I noticed i was having a lot of tightenings. They were like braxton hicks, but stronger and much more regular. Was put on the monitor at triage, which showed I was having 4 in 10 strong contractions. They couldn't believe they weren't hurting. They just felt uncomfortable and tight. They did an examination that showed my cervix was still closed and another actim partus and it came back a strong positive for risk of pre-term labour in the next 2 weeks. I was sent for a scan and my cervix had shortened slightly to 2.6cm. I was given my first dose of steroid injections and sent to a hospital 2.5 hours away from my home as none of the hospitals in my area could accommodate a pre 30 weeks baby. The contractions went on for about 48 hours in total and theb just stopped. I had another course of steroids at that hospital and then was discharged after a couple days. They put it down to either irritable uterus (but couldn't specify a cause) or practice labour.

At this point I finished work earlier than planned and just rested and pottered about. I was told to stay on the pogesterone until 36 weeks.

Then on Saturday night (31+5 week) the same regular painless tightenings started again. I went to bed hoping they would settle down, but they didn't so went to triage the next morning and was showing regular contractions again of 3 in 10 mins, but my cervix was still long and closed. I was put straight on to labour ward for monitoring as I was "threatening pre-term labour" and they prepped a bed in SCBU for baby just incase. The contractions continued until 4pm and then just stopped. No other symptoms of labour. I stayed in over night and was discharged the next morning (Monday).

The tightenings started again on Monday night, but were very infrequent so went to bed to see if they would settle on there own and when I got up they had worsened, so went back to triage and they weren't showing any real pattern this time, just spikes. Cervix was still closed and no other signs of labour so was sent home to rest and told to come back if contractions got worse or built up to a pattern or i had any other signs of labour. Again this has all been put down to irritable uterus or practice labour. They did say that they didn't think I would go full term, but could never be certain.

Today I feel fine and no tightenings. I'm really hoping that we can get to atleast 34 weeks.

Surprisingly I have remained relatively calm throughout all of this with the odd in hospital melt down. Baby has always been fine on scans and monitoring and I've never had an infection or anything. They can't say for sure whether everything that has happened is connected, but I'm assuming it must be in some way.

My question really is has anyone experienced these practice labours or irritable uterus as early as me. Did you have pre-term labour. How quick did it happen? I'm worried about these practice labours making real labour quicker and not getting to the hospital in time with a preterm baba.

Hi!

Didn’t have the discharge, but had the tightening. Took me about 16 hours between my water breaking for my son to be born.

I would pay a lot of attention to any “wetness” you’re feeling as this could be a sign of water leaking or breaking.

I would pay a lot of attention to any “wetness” you’re feeling as this could be a sign of water leaking or breaking.

Hello. Thank you for your response @Matthew54. Did you have the tightenings for long before you gave birth? What gestation were you? I've took to wearing a pad so I don't miss anything 🙈

Yes, I had some sharp pains and tightening at 34 weeks and then my water burst at work. It was pretty obvious for me then, but in hindsight I may have been leaking the day before.

I haven’t experienced the circumstances you have, but I was also under the pre term clinic (LLETZ and another risk factor) and my baby was born at 31 weeks. I was consistently told in the days leading up to labour that babies born at that gestation “tend to do very well” and happily that’s been the case overall. So it’s great you’ve come this far and I hope that’s of some reassurance (although you sound very calm and stoic about it all!). Best of luck OP.

Thank you for replying @Baloo592 that is definitely reassuring. I'm trying to stay calm as the midwives tell me that stress can induce labour. Trying to be as logical and practical as I can be. Did you have a long lead up to labour or did it come out of the blue? We're you experiencing any symptoms leading up?

I was anticipating a pre term labour but late pre term like 35 weeks and no scans had indicated any issues so my waters breaking very dramatically at 30 weeks was definitely out of the blue in that sense. And no real symptoms either.. I didn’t feel very well the night before but that’s it. No contractions until a couple of days after my waters had broken so it sounds like the background circumstances are a bit different. I had a 4 day wait between waters going and birth but I’m not sure that can be of reassurance as it’s all so individual and actually a long gap came with serious complications of infection etc. Hopefully now you’re at or around 32 weeks plus, you’ll be able to go to your local neonatal rather than a specialist one so that’s also good. Hoping little one stays put for as long as possible and it’s great that they have so far (and that you’ve had the steroids course).

I was the same, I don't think there was ever a good chance of me going full term. Just hoping ai can hold on for another couple weeks. Oh gosh, that must have been so scary for you. Was your baby okay? Yes, they've said that I could stay in my local hospital if I had her now, which reduces the worry a lot.

@TinyMouseTheatre we're doing okay. Still hanging on in there 😊 The tightenings have settled back down. Just the odd braxton hick, a lot of pressure and discomfort currently. I'm guessing the Pogesterone is keeping things going? The body is a mysterious thing.. 😕

Hi OP, how are you getting on?
I just read your post and wanted to share my experience as it may be reassuring.

with my first DD I had lots of episodes of tightenings from around 19 weeks onwards. No significant changes to cervix length. Went into labour at 38+2.

my second DD had tightenings from around 9 weeks! And then cramping at 24 weeks followed by a positive FFN and shortening cervix. Was admitted for a month and given progesterone and steroids. Made it to 39 weeks with this one!

no one could really explain the ‘tightenings’ - definitely seemed like irritable uterus from my own research but as you say the body works in mysterious ways!

Thanks for your reply @Glitterheart. That is definitely reassuring ❤️ I am thankfully still pregnant atm. Since my original post things haven't really improved symptom wise though.

Just after my original post I had a midwife appointment (33+2) and babies heart rate was elevated and she was also concerned about babys growth from her measurement. I went to hospital and monitoring was fine for baby, but a scan confirmed that although she had grown, it was not as much as expected. Im booked in for another scan this Thursday (35+3) to see how things are looking.

I was admitted to hospital for a couple days again at 33+6 because the tightenings had become regular 3/4 in 10 amd were painful and I had a 2nd positive actim partus, but again no change to cervix so was discharged.

The tightenings are a daily occurrence now and I will get a couple hours of irregular strong ones each day where all I can do is rest and wait for them to pass. They have become quite painful at times too. I can't walk very far or weirdly have a bath anymore becauseI get really strong tightenings that are so painful I can't move.

On Sunday just gone (34+6) I was in bed and had a gush of fluid that soaked through my underwear and on to the bed and I thought my waters had ruptured. Went to the hospital and was put on the monitor which showed regular tightenings, alot of which I couldn't feel (it would seem I can now only feel the 50 upwards ones now 🙈). I thought I must be in labour this time, but an examination showed that my cervix was closed and they couldn't see anymore leaking of waters and PROM test was negative. They said I could stay in, but that given my history if I wanted to go home and see how things went I could, so that's what I did. They said the gush could have been a mixture of discharge and urine as pressure can do that apparantly 🤷🏼‍♀️ I'm not convinced, but atleast i have a scan on Thursday to check.

So, that's where I'm up to basically. Currently 35+1. Midwife tomorrow and scan and consultant on Thursday, so will see what that brings.

I keep telling myself it's only another couple weeks at the most, but it's getting really hard. I don't want my mood to impact me in a way that it affects me when she's here. Luckily I'm a psychotherapist which has helped me stay fairly resisilient, but I'm defo having more bad days currently.

I'm sure everything will have been worth it once my first (and last!! 😂) baby is in my arms ❤️
