How do I get my maternity certificate

I meant to ask at my booking in and in the flurry of blood tests etc I forgot. I want to book dentist appt but presumably I need this to get it for free. Can I get it at my scan do you know? IS pain to go to docs just to ge this.

You get it from the midwife, usually at around 20 weeks. They won't do it at the scan, no.

Phone your midwife and ask her to print it for you and you then just go and pick it up?

Do you mean the NHS maternity exemption one? Your MW/GP needs to sign the form for you, so you'll need to see either of them. MAybe call your MW and ask her to put one in the post for you.

It's worth phoning your dentist to ask them about their policy on it. I've had dental treatment (from 15 weeks) and they said they were more than happy to just make a note of my due date and receive my exemption certificate at a later date. They were really good about it and aren't really bothered about when they see it. I had no problem with prescriptions either even though they should see a certificate for that too.

Check with your dentist - I just ahd to tell them I was pregnant and sign the dispensation form - didn't actually need a form from the midwife. Obviously depends on the niceness of your dentist though!

oooh you are right - I rang dentist and they said I can bring in certificate at later date. Yay! Now feel all organised.

I'm only 6 weeks and my doctor has already sent mine off for me. Although I am very luck in that my doctor's surgery is 2 minutes walk from my house and I work from home quite a lot!

I don't think you have to wait 'til 15 weeks. I've sadly, so far, never made it past 12 weeks but have always got my exemption card very early on by getting a form from my doctor.

the midwife usually sends for it at the first appt. I received mine through the post when i was around 11.5 weeks.

i got the form for the maternity exemption certificate from my doctors receptionist filled it in and asked them to ask the doctor to sign it ( she had already done my referral to the hospital so knew I was pregnant) she signed it out of hours no appointment needed and sent it off.

I just asked at reception if this was ok as i didn't want to waste the doctors time.

I got my exemption card from the doctor... that is, he filled out the form for me at 5 weeks and it arrved in the post a week or so later.

I am getting my MATB1 form (for maternity leave at work) at my next mw appointment (25 weeks).
