18 weeks pregnant pressure in ears, left ear crackling when I burp, yawn or swallow getting really fed up and anxious

Hello everyone
Just would like to hear anyone else going through this. This is my fourth pregnancy currently at 18 weeks, I have an 8 years old daughter and then had 2 recurrent early miscarriages back in 2022. So far the 12 weeks scan was fine waiting for 20 weeks scan coming up soon.
Since I found out I was pregnant end of Jan 2025 my left ear would randomly be blocked very uncomfortable my voice when I was talking sounded like a microphone or going through a tunnel this would last for 2-3 hours and then unblock itself and I would be fine with no lingering symptoms. However, this happened again the same ear in February, March, April ( during half-term). Then last Tuesday my mum put in a few ear drops my voice returned to normal as I could hear from my left ear. However, when I woke up on Wednesday and till now continuously there is pressure on my ears especially left like I have fluid trapped in there. When I apply slight pressure e.g. burp, yawn or swallow there is a crackling sound like rice krispies very uncomfortable, don't know if using the ear drops on Tuesday has made this worse. My hearing is fine and when I talk my voice sounds normal to me. However, the crackling has not stopped. The gp checked both my ears with otoscope and said there is no swelling, no discharge and no wax build up, eardrum looks fine. She has referred me to an ENT clinic my first appointment is at the end of this month. She said it could be some fluid behind the eardrum middle ear (but since I have been pregnant I have not had any viral infections or congestion) or it seems that there is a pressure imbalance in the Eustachian tube (ETD).

I did get a second option and asked the local pharmacy who do an ear checking service. He quickly had a look at both my ears and said there is no wax build up they are clean.
So I have read across some threads that this is common during pregnancy, but I never had this with my first pregnancy where I had severe morning sickness. This pregnancy I had no morning sickness and have been quite well until this ear business started. Prior being pregnant I have never had any problems with my ears even when I have been ill with sinusitis, viruses, vomiting bugs in the past my ears have never crackled and never blocked previously. Don't know if this is a pregnancy thing or by coincidence my Eustachian tube is playing up now whilst i am pregnant. The gp said ideally not to put anything in the ear as the canal looks fine. I have done steam inhalation with Vicks but this has made no difference. My nasal passages and throat seems clear to me.
Due to being the weekend the ear crackling was getting to me and I do suffer with anxiety so I am thinking this is how my ears are going to be for the rest of my life due to being so aware of the sensation and pressure I started to get dizzy spells like I am on a boat and i am swaying backwards, but I know this is stress and anxiety regarding my ears as when I am distracted like at work or with family the dizzy spells ease off.
Sorry for the long post just wanted to let it all out.
I was told by the pharmacist that if the crackling becomes constant like when sleeping or when just sitting, shooting pain in my ear that I am unable to eat or sleep then I should go to A&E and let them know what is happening and that I am pregnant.
But feel so upset as it is making me feel so weird like I am on an aeroplane all the time. Anyone had this whilst pregnant? Did it go away? What did ENT clinic suggest?

Anxiety is making my symptoms worse and making me dizzy. But very hard to distract myself especially with the crackling ears😥😢

I had problems with my ears each time I was pregnant. My ears would pop (like when you are on a plane) all day, everyday. Talking - I would sound as if I had a heavy cold. I was referred to ENT during my second pregnancy and was basically told that pregnancy does weird things including to the Eustachian tubes and there was nothing that could be done.

Thank you for your reply. Sorry that you went through this each time that you were pregnant. Did the symptoms ease off after having your baby. How did you cope?
I am trying my best to ignore it as it's not painful thank god, just uncomfortable, pressure build up with anxiety dizziness

I had the same thing too when I was pregnant. It sorted itself out after the birth. I do have issues with my eustachian tubes, dizzy spells and ear wax build up though and have to have my ears microsunctioned every 6 months or so.

Yes, it went away after birth each time. As to coping….i just put up with it I suppose. It became my new normal. It really wasn’t fun though so I do sympathise. Nothing helped though I did spend much of my pregnancy hanging upside down off my bed which unblocked my ears for a few seconds at a time. Good luck - I hope your symptoms settle down asap.

Sympathy for you OP - my left ear went crazy when I was pregnant with my first too.

I was sitting on the sofa one day about 26W pregnant then pop! Couldn’t hear a thing out of my left ear. Went to ENT clinic, had exams etc. but they couldn’t explain why. Even after a course of steroids injection straight into the ear drum, the hearing only came back about 20%. Luckily it slowly got back to normal a few months after birth, but I can’t tell you for sure if it’s 100% or not (haven’t heard back from the hospital for follow-up check up 2 years later).

Hopefully it’ll go away for you too eventually.

Thank you all for your messages. I am reassured that I am not the only person to have this whilst pregnant. I would not want this to happen to anyone else, but at least I know that I am not going crazy with my ears during pregnancy..... How bizarre, someone did mention due to increased blood volume and pressure during pregnancy congestion and changes can occur in the ears.
Well I had to rule out ear infection, ear drum and wax build up by the doctors. If symptoms get worse then will get checked out.
For now will manage and live with it somehow ❤️

How did you guys manage with a closed up ear. Today everytime I swallow or blow my nose my left ear closed up so my voice sounds like I am talking through a microphone. There is no pain, but it is very uncomfortable and when I swallow by closing my nostrils the ear eventually opens up but this is happening numerous times and when I keep pinching my nostrils shit to open my ear it is making me dizzy as I am getting tired. Should I just leave my ear alone 😥 rather than forcing it to open. This is all making me nauseous.

Not sure if this is helpful as it's not pregnancy related but I get the popping and microphone / tunnel sound thing when I'm dehydrated. I sympathise as it's horrible. No idea if this is the root cause but if it is it won't go away just by drinking a glass of water, you'll need to rehydrate over the course of a couple of hours to feel the benefit.

Thank you, I will make sure I am hydrated during the day. I am drinking water throughout the day. But this has become a vicious cycle when I fixate on my ear I start to feel dizzy and sick which is feeding into a cycle of anxiety. I don't know how I can ignore this, if I had a couple of days of respite, but it has been continuous since the past 6 days. There are people who have said they had this throughout the reminder of their pregnancies, but just learnt to cope with it.

One thing which I only found out about afterwards and which still helps me with this is a nasal spray. I use Sterimar hay fever and allergies one but I'm sure there are plenty. It fixes a bit of the eustachian tube dysfunction, at least for me. Chewing and swallowing helps too as well as yawning. For me it's definitely made worse by wax and that causes dizziness too so if I were you, I'd possibly get your ears looked at again to double check.

Well after having some respite for 9 days or so the left ear has been blocked today on and off 5 times. Will randomly open up and then close. No matter what I do the left ear opens on its own accord when it wants to and will then close up. I have my ENT appointment in 2 weeks time. Spoke to community midwife she never heard of ears blocking and Eustachian tube Dysfunction whilst being pregnant. Having said that I am 19 weeks the nausea and dizziness is bad and then having this happening is just an anxiety trigger.
Don't think olive oil drops will help as ears have been checked twice and on both occasions been told no wax. Steam doesn't do anything and pinching my nostrils is just causing me to be dizzy. When will it end 😭

Why does this happened during pregnancy?
I have never had this before even when I was not pregnant. My last pregnancy which was 8 years ago with my daughter I had severe reflux and morning sickness was vomiting non stop but I never had ear problems like this.
In December and November last year I had stomach bugs and COVID and I never had this.

This sounds like silent reflux to me. Look up silent reflux ear symptoms (there are other more common symptoms of silent reflux so the list of symptoms doesn’t always include the ones involving the ear). ENT should be able to confirm but in the meantime if you think it fits, you could try Gaviscon. Gaviscon Advance is the best for silent reflux and it’s ok to take it in pregnancy.

Thank you for your message. I will have a look. So do u try and tackle the silent reflux? So the ear is secondary symptom maybe because of silent reflux?

GP did mention gavison, but have been referred to ENT.
Could be silent reflux with severe health anxiety which are affecting my ears. Having said that in the first trimester I had no sickness just weird taste in my mouth and I was eating ok, but ear was still getting blocked, but now it is happening after every 6 to 7 days it's like my brain is waiting when will it happen again which is not good as my mental health is taking a nose dive.

Yes, essentially. I’m not pregnant but was having very similar symptoms to what you’re describing and since gastric reflux is common in pregnancy I suspect that silent reflux is too. It’s where some of the gastric juices are getting into your throat where they cause inflammation and the body produces excess mucous in order to try and protect itself. So your ears, throat and sinuses can feel very congested. You don’t feel any gastric symptoms. Hence the term silent reflux.

You can consider dietary and lifestyle changes - they might help, but since you’re pregnant there will be extra pressure on the lower oesophageal sphincter so it might not be sufficient for you at this stage. I was prescribed omeprazole but I couldn’t tolerate it so I’m just taking Gaviscon (at bedtime) now.

If you think the description fits, there is a Facebook group called Strictly LPR/silent reflux support group that you might find useful - there might be someone on there who’s suffered during pregnancy and could give you some tips.

Thank u will have a look. GP did ask if I have been ill and I haven't no flu or sinusitis, but soon as I found out I am pregnant this was been happening. There are a couple of threads I have come across of people who where pregnant and had similar issues with sickness or nausea, but after pregnancy their ear symptoms resolved. Just hoping I get some respite soon

I sympathise. The ear congestion and pain drove me crazy!

I’m much better now so there’s hope for you, if it is reflux!
