V Early pregnancy, woke up in pain

Hey, so I’m about 3+4 according to lmp. I had increasing abdominal pain yesterday and some brown discharge on wiping once yesterday and a little again this morning. I woke up at 3 feeling nauseous and in pain. Definitely sore all over but also on lower right side. It’s confusing. I’ve had pain from bowel movements this week which is v unusual for me, and that was certainly the case this morning, but it keeps coming back even after relieving myself. My pregnancy test is very slightly lighter this morning but it was the middle of the night so I’m willing to not focus on that.

im going to call epu as soon as it opens, but im worried this is ectopic, and an emergency. Or not at all?! No shoulder tip pain. And I have been a little gassy. I’m basically scared and sad and looking for advice or reassurance. History is 3.5 years ttc, 1 mmc, two failed ivf rounds, and now this.

I’ll add, the pain feels too constant to be cramping. During my miscarriage there were moments of respite. Though this is way earlier than that.

Oh and also although my test is lighter it’s still pretty strong. I’ll add yesterdays and todays here.

They are incredibly strong lines for the dates. Did you ovulate very early in your cycle? Most people at 3.4 are only about 11dpo.
I’m not sure even if the EPU see you that they’ll be able to see anything that early. I don’t even think a sac would be visible at 3.4.

Nosejug · 11/04/2025 07:27

I am about 15 dpo I think. I also had a positive test extremely early at 6dpo. (I was testing and can pinpoint my lh surge within a few hours) I know how unlikely this is, and in fact it was flagged as potentially ectopic by the fertility doctor who was about to do out next ivf, but the epu said from my symptoms/non symptoms they weren’t worried about that. I have a scan in two weeks with them, but they said to call them if anything changed so I’m sure they can analyse for symptoms of ectopic even if it’s too microscopic to see. Luckily they open in 33 mins!

I agree that those lines are much darker than I’d expect to see at less than 4 weeks!

I had an ectopic with my first and had brown watery discharge around 4 weeks which stopped, and then reoccurred at 7+3 alongside one sided pain.

They don’t tend to scan until 6 weeks because they can’t see much! But they might monitor your HCG levels for clues. In ectopic pregnancies HCG doesn’t tend to behave in the same way and if you are having one sided pain they might do it for you.

I will say though that I have had one sided pain in both of my healthy pregnancies that have followed and it’s just been where a cyst forms on the ovary I ovulated from (normal).

Funnily enough @heroinechic they did find a small cyst on my right ovary! They tried to see pregnancy but it’s likely just too early. They took bloods and will again on Sunday and then I’ll have a picture of how things are right now. I’m v lucky to have such a supportive epu ❤️

@Nosejugit’s fantastic that they’ve taken you seriously and are monitoring ❤️ all the best with your pregnancy x

Nosejug · 11/04/2025 16:17

Thanks @heroinechic :) I got a good report on my blood homework so as long as things are doubled by Sunday I am going to assume my digestion was to blame and just ignore the blood unless it’s actively bleeding. Quite relieved!

That sounds good! Fingers crossed for Sunday bloods 🤞🏻

Thought I should add for anyone reading in the future (I know for sure I scour Mumsnet threads for clues with what’s happening with my pregnancy/body) that it was indeed an ectopic pregnancy. Strangely the pain still comes and goes, I have two small cycts on my right ovary so the pain could be that or the small heamorrhagic areas in my abdomen/on tube. I’m on expectant management after nearly having my tube removed, a delay in the surgery from someone in greater need than me meant more blood tests showed my hcg was beginning to come down and my tube can maybe stay with me. Still in danger zone but lower in the danger scale.

its not been ruled out that there could have been two pregnancies (two cycle, early positive, early high hcg and very high progesterone 109!) but either which way I’m not getting a baby. Just want to get back to trying again!

good luck to anyone reading this and going through similar x

*two cysts, not “two cycle”

So sorry to hear that update @NosejugI also had expectant management rather than removal though it was touch and go in the moment! It’s such a complex feeling: devastation that the pregnancy isn’t viable vs relief that you aren’t requiring emergency surgery and get to keep your tube.

Will they do any investigations for you? They wouldn’t for me but might for you considering your history.

I haven’t had any lasting issues except ovulation pain on my left side whenever I ovulate from that side!

Nosejug · 23/04/2025 10:37

@heroinechic its funny, the sadness of pregnancy loss was quickly overshadowed by the medical side of things. My mmc was in its way beautiful, that the wrong word maybe but something like that. This time round it’s waiting in a room fasting for 6 hours every two days while we wait for bloods to decide what’s next… ! But yes I’d be happy with expectant management to continue. Do you mean investigations like blocked tubes? They were going to (is autopsy the word?) investigate my tube when it was taken out but then I’ve avoided that for now. I’ve had a clear hycosi before so there was no known blockage, but a myriad of things can cause it. I’ve been reading into oil based hsg X-rays which can wildly increase spontaneous conception and imho would clear any blockage post ectopic, but nhs def won’t offer me that, as they said no to second hicosi. My next step is last and final stop ivf anyway, we were due to begin when I got my bfp. We’re having our embryo(s) tested this time to check if they’re viable or not, and if none are viable then I’ll accept my eggs just aged too much. We have had councelling to consider donor eggs and I have more thinking to do on that, and a little time to do it. So I guess that’s the plan overall, but maybe if my embryos are exploding then it’s worth continuing ttc naturally for a short while if ivf doesn’t work. Sorry that answered way more than you asked!

OP posts:
Nosejug · 23/04/2025 10:39

Haha not exploding?! Euploid. Please god not exploding embryos while I’m going through ectopic.
