Large painful cyst early pregnancy

Hi all, I wanted to see if anyone has similar experiences. I’m currently 6.5 weeks pregnant and I was experiencing a lot of pelvic pain and urinary retention. Was worried it could be ectopic, so booked an early scan. It turns out the pain comes from a 7cm cyst in one of my ovaries. The sonographer said they will keep an eye on it but nothing to do at the moment, unless it gets too painful, which could indicate torsion.

it is very uncomfortable right now , though ! Maybe because I have a retroverted uterus as well?

has anyone experienced a large painful cyst this early in pregnancy and what was the outcome ? I’m really hoping in shrinks.

I had a huge cyst at that grew to a size of a football with my first pregnancy and currently have two ovarians cysts both 12cm wide each. I’m lucky to have minimal symptoms though but was told by my consultant to present myself to A&E if I have abdominal pain. Make sure you’re consultant led during your pregnancy!

Thank you @cosymama2020 ! Did your cysts grow during the pregnancy ? I went to the early pregnancy unit, and they said I should go to A&E if it becomes very painful. Right now it’s mostly sharp pains down there, bloating, and the bladder pressure.

I had the same - a 10cm cyst when I was about 8 weeks pregnant - they left it for a while as they were worried it was the corpeus lutum which sustains the pregnancy until the placenta takes over. I was sure it wasn't as I'd had some random pains in the area before pregnancy but they didn't want to take the chance. In the end I got admitted as the pain was too bad and after a week of umming and ahhing they said they were going to operate then at the last minute drained it under local with a huge needle (it didn't hurt). It took a few more days until the pain went away as stuff needed to move back into place but after that it was fine and a scan after DS was born showed the cyst sac had disssolved.

I'm not sure a sonographer is qualified to make the decision of what to do next? I'd go and see your GP if I were you.

When I was pregnant with dd1, I had abdominal pain early on and it was discovered that I had a dermoid cyst on my left ovary. Apparently, I had had it my whole life but it had never caused any pain until I got pregnant 🤨

Anyway, they said I may need a laparoscopy if it twisted or ruptured and I was on consultant led care. Also meant I had to have a C-section. At which point they had to remove my left ovary as the cyst was so large and I was heading towards a blood transfusion otherwise.

Needless to say, the recovery was horrendous. Very traumatic. Took me a good year. Chances of getting pregnant again down to 70% (rather than 50%) with just the one right ovary remaining.

Touch wood, still managed t9 get pregnant again 😉

Hopefully your situation is different as not a dermoid cyst, just a normal liquid one? But it's good they are keeping an eye on it.

@Titsywoo Thank you, I’m glad it all went well ! I wish they could do something about this cyst. To be honest I’m not sure if they are sonographers or nurses. they did my scan at the EPU and they seemed quite knowledgeable, although I’m concerned about whether I should get a second opinion. I haven’t had my first midwife appointment yet.

@Meredusoleil that sounds horrible ! I’m so sorry you went through this :/ but so glad you could get pregnant again . Mine is a fluid one, but they said it’s not the corpus lutem.

I'd definitely go to the GP and ask if it is advisable to see a gynaecologist about this and you are concerned about it growing and further pain. I think as so many cysts disappear in the 2nd trimester they like to watch and wait. Have they given you a follow up appt? If so then they will probably re-measure and figure out what to do based on whether is has grown or not. I used a hot water bottle for pain plus cocodamol but you should check if either of those are safe if you want to try them (this was 17 years ago for me and things change!).

They just said I should wait for my 12 week scan now (which is in 6 weeks). But you’re right, I think I’ll ring my GP to see if I can get a gynaecological appointment.

@Ninabd yes mine grew to be absolutely huge and needed to be removed after a C-section. I had no symptoms or pain though but my cysts are very rare and have a different fluid inside them than most other cysts.

My hospital take it very seriously and have taken it seriously again this time round so I’d call your GP and ask them to refer to write to your hospital to request a gyno appointment. Please ensure your pregnancy is consultant led as well! I had to have fortnightly scans and constant growth checks for both baby and the cyst. Everything was okay in the end with my first baby though!

I have bilateral endometriomas. They’re fairly large, but I have minimal symptoms (they were an incidental finding).

I’m 8+5 now. At my booking appointment the midwife put me under consultant led care because of the cysts. Hopefully yours will do the same and then you can clear up any concerns you have about cysts and pregnancy.
