High liver enzymes

Did anybody have high liver enzymes AST and ALT in early pregnancy (first trimester) and beginning of second? I had them high at 7 weeks (AST 66, ALT 109) and then I stopped prenatals per OBs instructions, and they lowered, but still high to AST 49 ALT 74, and now, four weeks later at 15 weeks pregnancy they are back to high (AST 63 ALT 129). My bilirubin and GGT are in normal range. My BP is also normal to low.

I don't have any itchiness nor any pain in upper abdomen, and my blood pressure is low. I've never had any liver problems nor anybody in my family.

Could it be early cholestasis? My OB is sending me to an abdominal ultrasound on monday, but I'm scared what could it be.
I'm googling early preeclampsia, liver problems, early cholestasis, early helllp syndrome.
Its my first pregnancy and I'm 38 years old, will be 39 when I deliver.
And I'm super scared.

Any tests I could do?

I had this along with dodgy kidney ones. I had to go in for blood tests and monitoring every three days. Cost me a fortune in parking. Was all fine. Had to have it rechecked a few weeks after birth and it was back to normal

Hi @ParanoidAndroid39
I had this in my first / second trimester and had the abdomen scan. I'm a slim build but was having alot of full fat yoghurt and cheese. I cut back ahead of my scan just in case it was that. All turned out ok at the abdomen scan. Not sure why my bloods were at that level but all was ok. Try not to worry hun, hoping all will be ok for you too xx

Thanks! I've been eating some full fat food (dairy and fried meats) and less veg than usual in the first trimester - cause of my food aversion/sickness. Now I've stared to eat more soupy meals, I hope it will show in my next test. And its good to know that at least for you it amounted to not a big deal :)

Do you remember which week of your pregnancy it was, and what were the indicators for kidney troubles? Maybe I could check that too. I'm game to check anything that could be related to this, just to act pre-emptively :)

I had this up to about 250 from my booking appt to my mid second trimester then they just started dropping again and no one knew why. My consultant suggested a general viral infection (like a cold) can cause this in pregnancy.

ParanoidAndroid39 · 07/10/2023 17:26

@jamontoast2
thanks for the info :) I hope they will start dropping, same as you. Its a bit easier now to wait for my appointment and ultrasound on monday. I hadn't had any viral symptoms, but my GP also said it could be a HEP A infection, or a viral infection of some sort

@ParanoidAndroid39 can't remember sorry. Was definitely third trimester. It was just another thing after hyperemesis and SPD. I didn't do pregnancy well. I think there was a migraine type headache when it was first picked up too

Yeah, an obstetrician I talked to said a virus could have caused mine to go wonky early in my second pregnancy. I also have a history of high liver enzymes when it was found incidentally in a blood sample taken when I wasn't pregnant. I'm afraid I can't remember what week it was, but at the repeat bloods it was all back to normal.

However my liver enzymes went slightly bonkers in late pregnancy (both pregnancies) at 36 weeks, and that was pre-eclampsia. With my second it went so high they sent me for an abdominal ultrasound just to make sure my liver was ok. It was. I just think my liver is a bit over sensitive and throws tantrums overreacts to minor issues. Something it and my now toddler have in common. Both my babies and I were fine. They were good sized, and I had no other symptoms other than the BP, protein and liver enzymes.

@Dyra Thanks! I'll ask my OB if this means I'm at an elevated risk for preeclampsia and choleostasis in later weeks. Maybe he'll monitor me more often. My GP thinks it could have been a virus, but I wasn't sick that I know of. And (un)fortunately I don't have a history of liver issues..

I know it’s been a while but can I ask how this turned out for you @ParanoidAndroid39 My levels have just come back high at 25 weeks
