Giving birth after PPH in first pregnancy

Hi there. Hope you're keeping well. We're expecting dc2.

I had high blood pressure with dc1 from about 34 weeks. It ended up being pre eclampsia during the birth, which was induced at 39 weeks.

I lost 1.1litre plus of blood in labour. Hubby said most of it was when I passed the placenta.

Has anyone else given birth after a PPH? Or similar circumstances? What was your experience.

Pph first time round, due to womb being overstimulated by induction drip. Blood transfusion needed.
Second time was given drugs as precaution during labour lost 450ml, do you have a consultant appointment to discuss it? X

I'm figuring you want honest answers, so I'm reluctantly providing one. I had a PPH with one baby of about 1.5 L. The next I had a PPH of about 4L and had blood transfusions. I decided no more babies for me.
By way of encouragement, the staff will be aware of the risk and will actively manage you the next time. Depending on the cause of the first bleed, you may not have any risk of a repeat performance.

I lost 2L first time round, induction and fast birth and severe tearing. Opted for a section next time and still lost double what they expect for a section (1L). I think being anaemic is a factor, how's your iron?

You should be referred to a consultant to discuss, to summarise what I was told:

  • home birth/ midwife led unit not recommended
  • delivery room will be prepped with oxytocin drip, cannula to be put in place in final stages of delivery to recieve drip if required.

I PPH'ed again with dc2 but not dc3

First birth preeclampsia diagnosed at 36 weeks. Induced at 36+4 and had baby. Placenta accreta found and surgery failed to remove it. Balloon inserted to control bleeding after losing 4 units of blood and having 4 blood transfusions. Placenta fell out at 10 weeks post partum after contractions at home. Massively lucky I didn't get sepsis.

Second pregnancy thought ectopic but found I had a bicornuate uterus. Consultant led. Bleeding all through pregnancy. ELC accepted. Consultant referred me to other hospital for specialist placenta scan at 30+3 weeks to try diagnose if placenta accreta again. Didn't make it there. Baby suddenly arrived at 29+4 by spontaneous labour. No ELC as too risky due to not being far enough along. The placenta accreta did not return but placenta was stuck and surgeon successful removed it manual with spinal block. Only slight loss of blood so day after baby arrived had transfusion due to low iron.

No more kids now. It's obvious my body cannot carry them properly.

Oh and due to previous 4 blood transfusions, the hospital got 4 units of blood ready for me and kept close by incase I needed during second. Very prepared.

PPH with first
With my second I was able to labour in the pool but got out to push so I was already out if it did happen again. It didn't and it all went smoothly with number 3 and 4 as well.

I had an APH with my second, I'd had placenta praevia but it had moved out of the way by the time I went into labour. Was just monitored closely and they broke my waters when there was finally room on the delivery suite. Had other issues with my 3rd but this was never mentioned.

Depends on why it happened. I had along labour with the drip and a back to back baby plus low platelets during pregnancy then PPH and offered a transfusion. Second labour was quick, not back to back (thank you spinning babies!), but still low platelets and weirdly lost less blood than average. Third was induced but no drip and not that long compared to the first but I still lost almost as much blood as the first however felt so much better after. All high risk labour ward births and not allowed a pool because they needed to monitor blood loss.

I'd have a chat with a senior midwife about why you had pph first time round (you can request this from about 35 weeks here) and they can help you prep to minimise the risk of it again. I was also referred to haematology and had various consultant appointments throughout.

Personally I think the things that helped for me in recovering were being active during pregnancy so I was stronger going into recovery, having a load of help after, and eating really healthy food for weeks. The things that seemed to reduce the blood loss were knowing what I was doing so effectiveness of and therefore time taken pushing, position of baby, readiness for labour, and that bloody drip.

It's not guaranteed you'll have another one and it's not guaranteed your recovery will be as bad again if you do.
