Cervical length: 26-27 mm at 18 weeks. Is this ok?

I had the cervix measured today and it was 26 mm long. The sonographer said that she always under measures it and therefore its real measures would be higher. She was happy with the outcome and also added I would not need to repeat the measurement again. I would have thought that it would be better to repeat it, but probably that won't be possible. Any similar experiences? Thank you

So anything 2.5cm and below warrants treatment progesterone / cervical suture or both.

2.6 is close to the cut off so I think they should be scanning you again in a weeks time two weeks max.

Are you under a consultant ?

I had a stitch placed at 13 weeks as I had my last baby at 28 weeks. My cervix measured 2.6 at 10 weeks.

Thank you. Yes, I know that group. May I ask how was your experience of delivery at 28 weeks?

Ah, I forgot to say that I am not under a specific consultant. I should ask my midwife to do refer me, right?

I think you should push to be scanned again in 2 weeks. Mine was 2.4mm at that gestation and I started progesterone, it remained stable until I got to 22 weeks when they stopped scanning and started prepping for an early delivery instead. I had DD at 34 weeks in the end. I think yours is borderline enough to warrant another look, I found my hospital was quite blasé about my cervix shortening and I had to push for the scans and progesterone despite having an abnormality that is linked to pre term labour.

InpatientGardener · 19/08/2021 14:28

What prompted them to do cervical length scan? I guess depending on what that reason is you may or may not have a consultant.

sapo2000 · 19/08/2021 14:34

Thank you. I had a LLetz procedure in 2018 and 14 mm were removed. I
When more than 10 mm are removed, the pregnant woman should be offered a cervical screening. I had to fight a lot to get this booking though. My midwife at the pratice and gp knew nothing about this guideline.

Thank you. May I ask when/month did you have that 2,4 cm?

So mine was due to natural ic that went undiagnosed. He was born very quickly so had no time for steroids to work. He spent 108 days in nicu and has life long conditions from bleeds on the brain
That's why when we had our 2nd ds I was on it and made them see me very early on (10 weeks) the standard is 16 week, and with help from progesterone and a stitch carried him till 39+1
I am very passionate about incompetent cervix as no one should have to have baby's prematurely or loose their baby (sadly many lady's have late losses). When a couple of simple scans can arm lady's with what's happing with their body's am able them to make informed choices about the care they receive

Sadly care is very different depending on the hospital your are under, some are brilliant some are very poor

annlee3817 · 19/08/2021 15:28

I'd also be pushing to be scanned again, I was put on progesterone pessaries at 24 weeks when my shortened from 3.4 to 2.4 in just a week, all was fine in the end, bed rest at 32 weeks until 36 weeks as cervix was too soft and baby engaged, but I ended up going to 40+5 despite starting to dilate at 37 weeks :) I used the progesterone pessaries until 36 weeks

sapo2000 · 19/08/2021 15:50

Thank you. Is it correct that cervix gets significantly shorter after 24 weeks?

InpatientGardener · 19/08/2021 15:54

I think it was around 18 weeks. The next scan I had said it was 4.6mm and I was almost discharged but fortunately my consultant looked at it and saw they measured totally wrong. So it really can depend a lot on who does the scan. Is this your first baby? I found that because I was an unknown as a first time mum that they were less keen to take action because there was no evidence of something going wrong before. I would ring the antenatal department of your hospital and explain the situation, rather than bother trying to go through your community midwife.

annlee3817 · 19/08/2021 16:05

I didn't hear that, I am sure that I was told it should be above 3 until you're nearing the end of.your pregnancy. 2.4 at 24 weeks was definitely a cause for concern and the consultant monitored it ever two weeks to make sure it wasn't shortening further

sapo2000 · 19/08/2021 16:08

Yes this is my first. I will try to speak to a gp to persuade him to repeat the screening in 2 weeks. My midwife is very low energy and I don't expect her to help me with anything. I asked her about this cervical check when I was 12 weeks and it was completely useless.

Thank you. In my case, it is 26-27 at 18 weeks. I hope it does not change that much

I wouldn't go to your GP, they're likely to just tell you to contact your midwife. Your hospital will have a number to call for the Antenatal department, you can give them a ring and explain that you're worried. My cervical length scans all took place at the hospital and were then reviewed by a consultant who would make the decision about what to do next. You've basically been screened out of that process and further monitoring by your midwife, which I don't feel is right given the measurement you had.

boymum88 · 19/08/2021 16:22

Agree nothing to really do with your gp. You need to ring your booking hospital request they scan you again and get in a consultant clinic to review your scans and history. Within the next week
Be polite but firm

sapo2000 · 19/08/2021 18:40

Thank you everyone. I will call the Gynae ward tomorrow instead. My experience with GPs in this regard has been frustrating indeed.
