Maternity Exemption Certificate for Dentist

Hello,
I've just had a cap fall off my tooth and was wondering how I can apply for a maternity exemption certificate for the dentist? I had my booking in appointment a few days ago, and annoyingly, this just happened today. Do I have to call the midwife, is getting one before my first scan possible?

I went to the dentist whilst pregnant (and barely a bump) with no form. I think I just mentioned it to the receptionist and the dentist filled a form in?

Similar with a pharmacy. I've never shown any proof.

My dentist has always asked to see mine so perhaps phone your dentist and ask their requirements beforehand just in case. Prescriptions I've never been asked but for some reason my dentist is very hot on it!

You get the form from your GP, most will ask you to fill your part in then the Dr does theirs and the surgery sends it off.

You then get a letter in the post with a certificate number. Dentist may accept you just saying your pregnant but it's best to have the number just in case your asked 😊

You should have received the form at your booking app. You need your official due date to fill in the form really so better done after your dating scan.
You can reclaim any NHS covered cost after you get your exemption through if you keep hold of receipts from the dentist.

I’ve always needed to show my exemption certificate. My midwife emailed it after my booking appointment in my last pregnancy

I received my form at my booking appointment. As someone who works at a dentist we rarely ask for proof. We simply tick the box that says the reason for exemption. It is on the patient if it is indeed true etc. Maybe chase it up at your next midwife/scan appointment.

I was given my exemption application form at the booking appointment for both of mine. I have always had to show them as well - even with a massive bump!

I just rang the antenatal clinic and they said I won't get one before my 20 week scan. Not sure what I should do? This sounds much unlike your experience!

My midwife sent off for mine at my booking appointment.
My appointment wasn't in person this time so she emailed it to me

You'd be better off contacting your midwife. If you have her number just send her a text as she's the one who will be doing it

Speak to your midwife, if you're in England it should come through by email after your booking appointment at the moment. The hospital wouldn't normally get involved with exemption certificates and are probably referring to your MATB1 (for maternity pay/allowance).

I haven't sent off for my mine yet but needed a prescription, so the pharmacist gave me a form that'll allow me to get a refund for the prescription charge once I've got the card. The dentist might be able to give you something similar.

As a PP has said, it sounds like they are getting confused with a matb1 form which you wouldn't usually get until around 20 weeks (and that's the proof you show your employer).

I got mine by filling in a form my midwife gave me - ask your midwife. While you are waiting for it she said you can show them your maternity notes folder if they ask questions.

I tried to speak to the midwife, but she didn't come to the phone. I don't have her direct number, and didn't want to bother her any further, so I rang the England maternity helpline number who confirmed I should be able to get one emailed to me. They took my email address and details and I'm hoping I receive it soon.

I think they've got your MatB1 and exemption certificate mixed up.
The exemption certificate should be emailed to you after your booking appointment.

Update: Just got my exemption certificate emailed to me. I found the midwife/ maternity helpline (in England) extremely helpful!

Can I ask what helpline that was?

I don't have a midwife yet as my booking was done at hospital and the exemption certificate would be really helpful to get my nausea medication for free. Been paying for it so far!

I'm sure you know this already but I wasn't able to register for an NHS dentist (no availability locally) so was paying privately before PG. That meant my exemption wasn't valid and I still have to pay.

I'm a dental nurse and we require no proof of your pregnancy.
We just need you to tick on the NHS form. I mean if anyone is lying the fine goes to them anyway. When we send your forms off to the NHS in the computer it will flag to them if someone has lied.
But you're pregnant, we don't need to see any proof. Grin
