Statutory Maternity Allowance- Self-employed

I'm slightly confused about which forms I should be completing and who I give them too. My midwife sent me the Maternity Certificate however as I'm self employed I think I will need to complete a MAT1 form?

I've had a look online and it's not making much sense. Can anyone in a similar position shed some light?

Do you mean she sent you the MatB1 certificate? You then apply for maternity allowance
www.gov.uk/maternity-allowance/how-to-claim you can do it online or print it and fill it in

And its an MA1 form you're completing. The MatB1 is your proof of pregnancy

That's the form I used that @dementedpixie has linked. I printed mine and filled it in as that felt easier. It's about 20 pages long but you only fill in a small portion of it.

Looks really complicated but wasnt too bad - the most annoying bit is the columns which you'll find as a PDF download on the gov website. They also only took a week to send my reply which was far quicker than I thought.

Thanks! It is quite an intimidating form but on closer inspection I see that only certain sections are relevant to me.

Do you know if anything is to be passed to my accountant?

@Keepingcomfy sorry can't help on that as I do my own taxes so I just filled it in myself. It just took all my details from HRMC for the testing period so you might be okay without but could be worth asking them.
