Cervical stitch 14 weeks

Hi everyone this is my first post but need some advice. It's a bit long so I apologise.
I'm 14weeks 3days pregnant with my 3rd child. I had a lletz treatment in April 2019 and initially the midwifes weren't going to refer me for a cervical length scan. After some investigation into my treatment in April I pushed to have this scan done. For some reason they booked me in for this today even though I'm not 16 weeks yet (apparently the recommended time) and my cervix was already only 22.8 mm. The midwife took my report to the consultant who wrote on it 'referral for emergency cervical cerclage' and I was then taken down to the EPU dept. Once there the Dr I saw wasn't happy that the scan had been done early and has requested another scan in 2 weeks and not to have the stitch done until after that. At the time I was a bit shocked and also on my own as no partners are now allowed to attend.

I guess my question is should I wait the 2 weeks or push to have the procedure now as my cervix is already shorter than the recommended length? Any advice or people who have experienced the same would be greatly appreciated 😃

Hey,

I’ve recently experienced similar, I’m in Ireland and so perhaps it’s a bit different here.
I had a LLETZ procedure in 2015 and shortly afterwards a surgical termination and D&C as the termination was incomplete. The consultants don’t believe my short cervix is to do with the latter two, just the LLETZ.

I was told after the procedure in 2015 that I may need a stitch if I were to fall pregnant again, so it is something I broached with the sonographer at the 12 week scan (13+1).

They did the usual ultrasound and once that was performed the transvaginal. My cervix measured 17mm at the shortest measurement and 24mm too - not good. The sonographer said she thought I would need a stitch and rang the consultant.

A few minutes later I was told that I was booked in for a preventative cerclage at 8am that following Monday (that was Friday early evening).

Had the cerclage, not the most pleasant experience - spinal anaesthetic and catheter when I couldn’t wee 🙈 But they said to continue as normal with the rest of my pregnancy - no additional scans were needed aside from a F/U on the Friday. No restrictions - maybe that will change but they don’t seem to think so.

The procedure was 10 days ago now and so far, so good. No pain, things seem fine.

It’s unnerving but I’m trying not to worry. They did say the cervix could stay at 17mm for the duration of the pregnancy without a cerclage, change to 24mm or to above the threshold - as the cervix is dynamic. The cerclage is just added reassurance.

If you are anxious speak with someone tomorrow and explain the anxiety. I understand how you feel and it is an awful worry.

I had no issues with my DD’s pregnancy (mind, it was in 2008), so this all came as a big shock.

Hi. Thanks so much for sharing your experience. I guess it's just a worry about the whole situation and not understanding why the EPU Dr wants to wait another 2 weeks when she told me that my cervix will not get any thicker.

Why risk the 2 weeks? I wish I hadn't felt so shocked at the time and could have my point.

Will try to ring someone tomorrow and get some further advice.

Understandable so with the difference in opinion. I would be the same.
I have no medical training but maybe 22mm at 14 weeks didn’t strike the EPU doctor as extreme enough? I really don’t know but I would query why two experts diverged so much in opinion and state it is a cause of anxiety.
Preventative stitches are usually placed earlier in the second trimester, but can be done up to 24 weeks, I think?
Anyway, at least it is on their radar and now you will be monitored. Hopefully today will bring a clearer picture, best of luck with it x

Just a little update.

Spoke with the antenatal midwife this morning and she immediately spoke with the consultant. He was very cross that his request had been changed. Had to go back in today to sign the consent and the procedure is being done on Monday.

I’m glad to hear it, it does offer a sense of reassurance and there’s lots of positive stories out there regarding the preventative cerclage, and remember, it could have been okay without it too -nevertheless, so grateful it was identified.
It is a scary thing to be told and the procedure certainly isn’t the most pleasant experience, but you both will be fine come Tuesday.
