Hi OP, didn't want to read and run. I'm on my 4th pregnancy this year, though not assisted, and have had it go both ways in similar circumstances, so thought I'd mention in case helpful.
First one was as you describe - sac, yolk but no fetal pole. It ended in a MMC unfortunately, confirmed after a week's wait, which showed no progress.
Second one was a suspected ectopic, had one in June so extra worried (it was gas! 😬). My hCG was, said the consultant, too high for what he was seeing on screen. I can't remember the numbers, not as high as yours, but hCG was about a couple of weeks ahead of what was on the screen - estimated at 4.5 weeks by scan. All that was visible was a black dot. I had to go back a week later and it was all good news, now measuring just right for hCG and showing progress. Just had my 12 week scan today and all is still well.
What I took away from all this is that measurements very early on are more of an art than a science. They are dealing with millilitres and the equipment can't always see everything. So there's a lot of 'wait and see'. With the suspected ectopic, I did slightly lose my nerve and asked the consultant to just tell me honestly if it's bad news, because I'd been there before. He said that, genuinely, it was too hard to say with these things without having two results side by side.
I'm afraid it's that very fraught wait and see week for you now. Everything crossed for you! I have to say I felt far more nauseous and sick with what is ny current pregnancy, even when stuff wasn't visible, so I think that's a good sign.
Btw, I'm sure you're googled everything already, but 37,000 is normal for 6 weeks, so not wildly excessive given where we think you are. And these things are only ranges anyway.
