Previous cervix procedure

I'm not sure if anyone on here can help me, I am almost 13 weeks pregnant and have previously (around 2 years ago) been treated for CIN2- abnormal cells found on cervical screening which involved pre cancer cells being removed from my cervix with a small op.
At the time of treatment the gynaecologist told me I would need to be closely monitored during pregnancy due to having a procedure on my cervix. I have explained this to my local midwife liaison who advised I may be under a consultant but since seeing my midwife for my booking appointment and having my 12 week scan nobody seems to have said anything about it...

Has anybody else had this kind of treatment? How has it affected your pregnancy/labour? Should I be pushing to be more closely monitored?

Thank you in advance Smile

Forgot to mention, I had LLETZ procedure

Yes. I have had exactly this kind of procedure (just one month before I conceived). I was also advised at the time of the treatment that if I fell pregnant again they might monitor me more closely and was slightly worried when it wasn’t mentioned.

My midwife said a weakened cervix due to scarring would be more problematic during early pregnancy (ie you’d end up miscarrying before 12 weeks). I saw a consultant after my 20-week scan for an unrelated reason and she confirmed that by that stage if the procedure was going to cause problems it would have done so by then.

Hope I’ve been able to put your mind at ease, as I do know how easy it can be to fret about these things. I’m full term now so would quite like my cervix to give way ;)

Having said all that, I do know someone else who had the same procedure as me about 2 years before having her first DC. It was never mentioned during first pregnancy but for some reason when she had her second they did have a look at her cervix to check all was ok, which was fine but she did think it was a little odd they hadn’t mentioned it during first pregnancy which was also closer to the time she’d had the treatment? So maybe it also depends on who you see and how serious they think it could potentially be?

Thanks @MagicMoneyTree that's really helpful! To be honest I had forgotten about it as scan was perfect etc but I just suddenly realised today that no one had said anything and I have mentioned it a few times!

Do you think it's worth mentioning when I have my scan in a few weeks?

I think it’ll be worth mentioning if only for the reassurance their response gives you. Assume you’ll have a midwife appointment at 16 weeks? No harm in mentioning it then and maybe again at the scan just to be sure they’re thinking along the same lines as the people I’ve seen.

In my mind, I was happier to raise it (when I finally remembered to) and have them tell me it’s basically a non-issue than for a problem to occur because they overlooked it and I hadn’t thought to mention it to them. Not sure if I’ve explained that very well but hopefully you get the gist of what I’m getting at? Xx

Lol yes I know exactly what you mean! I am almost 13 weeks now, next scan is at just over 14 weeks then midwife at 16 weeks so maybe il just both of them!
I think they may have put something about it in my notes but it was in nurse talk which unfortunately I don't understand Hmm

I've just had a cervical stitch put in for this reason- I had a LLETZ and cone biopsy procedures for cervical cancer last year and have been left with barely any cervix. I was told to call the colonoscopy dept as soon as I knew I was pg, was booked in for an early scan to check for viability and stitch was yesterday at 13+4. I'd insist they scan you!

PanannyPanoo · 04/01/2018 18:15

I had 8 procedures on my cervix including LLETZ and cone biopsys over 10 years before concieving. I am also hypermobile. I can't remember exactly but I think I have had 75% of my cervix removed. I had 2 consultant appointments during my pregnancy where it was scanned and measured and I had no need for a stitch and had no issues at all.

Definitely mention it at your next appointment, so you can have it monitored and a stitch put in if necessary. Congratulations!

Everyone is different, I had CIN2 and had the cells removed, had spotting on and off up to around 13 weeks. Very light, saw the consultant for something else so he decided to check cervical length at 21 weeks, then again at 23 it had shortened by a cm and was soft, so I had progesterone pessaries for the remainder of my pregnancy and was monitored. All was fine in the end though.

boodles101 · 05/01/2018 07:10

I had a Lletz procedure too but I think it was only minimal cells removed. I advised midwife at booking appt and she referred me to a consultant. I saw him about 21 weeks ish and was the same as MagicMoneyTrees in that he just said if there was going to be a problem it would have already been a problem. I was relieved but also annoyed in that they leave the appt so late where they probably wouldn't have been able to do anything if something did go wrong.
If I were you, I would chase the consultant appt, get seen and scanned so they can rule anything out. Don't wait for a problem to occur before they will see you.
My pregnancy was fine, and my DS was actually overdue. ( another thing I had read was about possible premature birth).

0310Star · 05/01/2018 07:31

Thanks everyone. I guess it's just worried me a bit after they said when I had the procedure that I would need to be monitored and now no one seems too fussed about it?! Il ask at my next appointment and see if they suggest seeing a consultant.

OP a single LLETZ for CIN2 is less likely to cause issues than repeated procedures or a cone biopsy unless you have a congenitally short cervix. However if it was mentioned to you at the time it should be checked. What part of the country are you in? There may be a local preterm prevention service you can self refer to. If not contact your midwife (and GP if unhelpful) and state that you have been told you are at risk of an incompetent cervix and that you need a cervical scan ASAP to measure length.

0310Star · 05/01/2018 08:56

@Paranormalbouquet thank you. They took a biopsy also but tbh I can't say I can remember exactly what was done! I think I may have wiped that awful experience from my memory Wink
Can a midwife access my previous doctors notes?

I had LLETZ for CIN2 two years ago. When I told my midwife at booking, she referred me to a consultant and they looked at how much tissue was removed (same hospital) on their computer records. If it's less than 10mm, they don't worry too much. I had 7mm removed but she still sent me for a cervical length scan which was fine.

december212 · 05/01/2018 15:37

I had two Lletz treatments prior to falling pregnant and was consultant led (by the same consultant who carried out the latest Lletz treatment) from 16 weeks. The 16 week appointment was really just outlining that my cervix length would be checked at the 20 week scan and any problems to be dealt with would be after that. As it happened, no problems and daughter was 10 days overdue.

Congratulations btw! It is scary after having previous treatments but it seems like most people go onto have normal pregnancies.

trevthecat · 05/01/2018 15:40

I had it done 2 years ago. Had baby 11 weeks ago. Was talk of a stitch in my cervix but didn't need it as it's only if it's Bern done 3 times. Had no extra care. All was fine. Baby born 1 day before due date

Bellamuerte · 05/01/2018 15:51

I had LLETZ - it only causes issues if the remaining cervix is too short. I had a cervical length scan at 16 weeks and the hospital decided mine was long enough to require no further treatment. If it had been short they'd have stitched it shut in a procedure called cerclage; the stitches are removed before giving birth. Remind the hospital consultant that you've had LLETZ so they can book you in for any necessary scans.

megletthesecond · 05/01/2018 16:11

I had 2 x LLETZ and my cervix was too scarred to dilate. We didn't realise this in my first pregnancy and it ended up in a near miss and emergency section.

If you've had problems with your periods coming out post LLETZ your cervix might be too tight. However I hope you have a safe and uneventful pregnancy Smile.
