Found an NHS dentist, but how do I get the forms I need for free treatment?

Can anyone tell me what form I need to claim free treatment during pregnancy and where I can get it?

You just need your NHS card that you apply for with the midwife for free prescriptions.

Mine also didn't ask to see anything but I have been going there for 17 years.

Yep, you just fill in the box stating exemption. The form you get from doctors is called an 'FW8', which gives you exemption from prescription fees. Your GP can fill it in, or your midwife. This also doubles as your proof of dental fee exemption.

However, you don't need proof to get it free, but get into a lot of trouble if you aren't actually exempt!
