I know to take folic acid and vitamin d for the first 12 weeks. After that, is it better to stop or carry on?

Yes, that's right!

NHS advice is that you should continue to take vitamin D supplements throughout pregnancy and while/if you're breastfeeding. You need a 10 microgram daily dose of vitamin D, which is quite a lot so make sure you're taking the right supplement. There's no harm to continuing taking folic acid longer, it just isn't important. You can take a multivitamin if you want, but take one that's designed for pregnancy rather than being a general multi-vitamin. (This is because you shouldn't have much vitamin A when you're pregnant.) It will contain the right amount of vitamin D, so don't take that as a separate supplement as well.
