Changing Name On Midwife Notes

Hi all,
I was wondering if anyone had managed to change their name on their notes, I had to join a GP surgery in order to get started with midwives and filled out all the forms with my new name and provided with ID and deed poll in case but they still put all of my details through with my maiden name..... Now all my midwifes notes are in my maiden name and I would like it changed in case it affects anything like the birth certificate.
Anyone managed to change it? Any ideas the best way to. My midewife had filled in a lot of my notes before she arrived for my booking in and I didn't know my GP had done this until then. Angry

It won't affect the birth certificate.

But I think you should just be able to notify them they've got it wrong.

I managed to change mine! Similar story to yours…I changed GP's when I found out I was pregnant (should have done it two years previously when I moved!), kept everything in my maiden name. When I went to my first midwife appointment she told me that baby would be 'baby maiden name' rather than 'baby married name' in hospital, I knew this would upset my DH so I got them to change it then and there. They actually had to do it twice as they spelt it wrong lol. All they do is print out sticky labels and whack them over the top of your notes. Then I eventually went back to GP and changed it there so everything was the same!

Have you asked them? Why wouldn't they be able to correct an error?

There will be various different computer systems. The mw may well have gone off hospital which probably isn't same system as gp. You'll need to have your name changed on all the different systems. pain in the bum. Just ask your mw what needs changing where. Won't affect birth certificate though.

I didn't (largely because I've always used my maiden name) and it has been a long-lasting inconvenience. Both DD and DS were registered with my married name. My DS needed a minor operation when he was about a year old. All his hospital notes assume his surname is my maiden name (because he was 'baby maiden name' when he was born. I spent ages trying to get the records changed (he has never had this surname as far as I am concerned), but with little success. They would smile and tell me it was all sorted and then either (a) issue his next appointment card/prescription with the wrong surname or (b) not be able to find his medical notes because they were still filed under the wrong surname. Interestingly, the GP has always had the correct surname on his records...
