preterm prom, pph, group b strep... next baby?

Ds was a text book pregnancy but dd had a few complications. My waters went at 36 weeks (at work!), turned out I had strep so induced which was followed with a 1 ltr haemorrhage.

Does anyone have experience of a second pregnancy after these? Know if they are often repeated? If care will change?
I'm a little nervous. Dd was fine at birth but is delayed now, I worry her preterm birth was a factor

My first baby wasnt pre term. He was induced (overdue) I had an hemorhage in labour (over a pint) and then a pph which was another 1300mls, ended up in hdu due to heart going into shock etc.

Dc2 no pph at all.

Dc3 i had group b strep detected in that pregnancy and was put on iv antibiotics during labour, but again fine no pph or anything.

Now 28 weeks with ds4 and no gbs detected. I still worry about having a pph again. And care wise, I have been on shared care (midwifes and consultants) with all pregnancys after ds1 birth and when I have gone into labour I have the cannulars fitted as soon as I go into hospital, so they can prepare if any bleeding does start.

Sorry I cant help on the preterm front though. x

Hi whicway, my DD was born at 34 weeks after my waters went, cause not known but believed to be an infection which weakened the sac. (But not GBS or any other "known" infection as they swabbed for those and all was clear)

Consultant has told me that, if it was due to an infection as they believe, it is no more likely to happen to me again than any other woman. I am not having any extra monitoring as they do not think there is any particular risk of pre term birth again. Hopefully they are right.

I am now 34+2 with dc2... so far so good...

In your case since you know it was a GBS infection I would ask for regular swabs to be done next time, that way if an infection shows up you can get antibiotics, and you'd want to know for birth anyway.

Don't know abut the PPH angle sorry.

Have had two PpROM - 24 weeks and 17 weeks pregnant. Had two normal pregnancies as well. 2DC. No rhyme or reason....

Mini pie, that's interesting. At the time no connection was made to gbs. Could it have caused it? They just said it was unknown.

I've never met a consultant in pregnancies, I don't know if I would this time.

I didn't even know I had gbs until labour, no one had told me. I remember listening to them taking about a lady with it then being shocked to find out it was me!

Mumof3 I'm hoping the PPh was related to a stupidly managed induction. Straight to drip, turned up when I said not to which was followed by 0 to birth in 15 min.

I asked to see a consultant when DD was about 18 months, before getting pregnant again, to go through my hospital notes and see if they could figure out why I had PPROM and DD came early (I had spontaneous labour a few hours later) and whether it was likely to happen again.

They agreed. He was very helpful and pretty much ruled out various causes based on my hospital notes (lots of info I had never known before) - eg he said not incompetent cervix as they checked my cervix soon after waters went and it was long and shut, not irritable uterus as no contractions till after waters went, placenta looked fine, etc. He said an infection weakening the sac was the most likely option since I had a couple of small leaks in the day then full PPROM that evening.

You could ask for a similar review of your notes with a consultant? I found it very helpful. I actually had a similar review after I got pregnant with DC2, at about 18 weeks I think, and got a very similar answer.

As regards whether GBS could have caused the PPROM - I believe it could have in theory, however I know it's also more likely that you will get GBS after PROM, so i guess it depends on whether you had it already or or only got it after the waters went? You will probably have had swabs done when you arrived in hospital with broken waters (I don't remember them being done actually but they were!) so the results of those would show whether you already had the GBS when you came in.

Which, I also have said many times I believed my induction was a factor in having the blood loss during labour and the pph after his birth, as it was so fast. Although not as fast as yours but very fast for a first birth, and my following 2 children I laboured very very slowly (5 days with ds2) and (6 days ds3) blood loss was only 300ml, and 100ml with my following dcs.

Hmmm, I didn't ask but I was induced straight to drip. I presume therefore I must have been a bit dilated. My first induction was aggggeeesss with pessaries to get going. I'd been ill at 32 weeks and one doc thought they could see my sac, the other said it was an erosion. As 36 weeks is just early I hope I can still talk it through as I worry about being even earlier.

My induction was ridiculous. It wasn't my first, I said I could feel it coming, they said they didn't change it but it was obvious. I went from walking and chatting to crying like I was being disemboweled with one continuous pain!
