Where do I send MATB1 to get prescription exemption card?

Question in the title! Midwife gave me the form but with no information. I don't have an employer so just need to use it to get the card!

I got mine sent to me after my initial docs appt at 5 weeks i think cos its a form u just sign in docs .
Matb1 is just for mat pay i think cos I just gave mine to my wirk.

Ohhhh .. you think I'd know that being on DC3! I'll have to ask at docs then. No wonder mw was a bit Confused when i asked her!

you need to fill in a form to apply for it - FW8 form: www.nhs.uk/chq/Pages/941.aspx?CategoryID=68&SubCategoryID=161

The exemption card form is an fw8 form and can be signed by any midwife or doctor who has seen you in this pregnancy. They will have a stack of them at the surgery.

It's a different form from the matb1 completely. Usually given at booking appointment Grin

If you self employed or working for your partner you still need MAT1b to claim maternity allowance- I only found that out after I told my midwife I didn't need one as not employed!!!!

You don't get matb1 till after 20 weeks but can get prescription exemption form filled out and card sent to you from really early. I think I did it at 10 weeks

2 different forms. MatB1 after 20 weeks from midwife. Mat medical exemption from reception at GP surgery.
