Im not sure if this is the best place to post-point me in the right direction if not!

I'm off sick at the moment with SPD but would like to go back to work but with reduced hours/working from home. HR have asked me to send a formal request in writing but I have no idea where to start Hmm anyone help?

I have no idea from an HR point of view, but you can get a fit note from your doctor stating reduced hours or working from home ... just a thought as this might be easiest?

I am going back to work on reduced hours and my manager required a letter from my GP detailing for how long I am allowed to work and what duties I can do - so I have a sick note saying I may be allowed back to work, providing my hours get reduced to 21 a week, I don't have stand any longer than 15 minutes and I am not asked to do any lifting.

As has been said - the GP can specify their instructions about Reduced Hours etc on their sick notes. One thing to be aware of though is check how/if it affects your pay. A year ago I had to take 6 weeks off sick (issue with my heart) and was obviously on full pay. After those 6 weeks I told my GP I felt better and ready to return to work but asked about reduced hours which he agreed to and set out a regime for me which basically consisted of me building up my hours over the next month until I hit my usual amount. However, doing this meant that I had to use my Annual Leave to cover the hours that I wasn't working which meant by the time I was back on full hours I'd used up nearly two weeks worth of Annual Leave. It was pretty shit.

I was told by some staff that I should have just stayed off on Full Pay and only returned to work when I was 100% ready and able to return to my normal hours as obviously the Annual Leave issue would have been avoided.

I'm off sick now with pregnancy issues (heart related again) and have only worked about 6 days since I was 10 weeks pregnant. I'm now over 31 weeks pregnant and have been signed off permanently, well, this was done when I was 24 weeks pregnant.

I obviously don't know how things work in your employment but just wanted to advise you to look into any potential issues with reduce hours.

Oh, and to add to what writer said, see how far along you are!

I've been off sick on full pay and of course my wage is going to go down now that I'm returning part time, but I was on full pay during the qualifying period for maternity leave, which can be really important.
