When you get it (20 weeks I think) you photocopy and give it HR. Job done. Smile

What penguin said,though you don't absolutely have to hand it in until 15 weeks before your due date,just so you know incase you haven't told anyone you work with yet.

Have you told your employer that you are pregnant? Check your maternity policy at work if you have one or speak to your manager/HR department but don't panic! I'm on second DC and didn't get my MATB1 until about 28 weeks this time as had a lot less MW appointments. However I had notified my employer in writing of the pregnancy prior to this.

If there is a policy in place they may require you to complete a form to notify the employer of your pregnancy but you can usually follow it up with a MATB1 later on.

If in doubt perhaps notify your manger in writing by email and let them know you will get your MATB1 at your next MW appointment then ask your MW.

Normally you need to notify before your 25th week (the end of it I think). My hr wanted the original doc so make sure you take a copy as your partner may need a copy for work as well.

Have you notified work of your pregnancy and planned leave dates? As long as you have, just chase your midwife for the MATB1 at your next appointment.

If you ask your MW she will give you a copy. Some trusts dont automatically hand them out. If you are in the UK, you need to notify your employer you are pregnant no later than 15 weeks prior to your EDD and normally they want the MatB form on later than 15 weeks prior to your EDD?

Ask your HR department what they require, most will want the original so keep a photocopy at home for your records, they'll probably want you to sign an in house form detailing your due date and when you plan to work until, when you plan to come back and to agree that you understand the maternity pay structure and your employment rights. It's on the understanding that those date can change, so don't worry about sticking to them.
