Episiotomy scar and 2nd pregnancy - TMI warning!

Hi, I don't know if this is normal or whether I should speak to a midwife. My next appointment isn't for 4 weeks.

I'm currently 30 weeks pregnant with DC2. My bump was measuring 1 week ahead last time I was measured but I am only short (5ft 1) with a small torso and I look huge. Not a day goes by when someone doesn't ask me if it's twins.

For the past week, I've been itchy and sore in the vagina region. Not the opening but more so the labia. I haven't had any unusual discharge so don't think it is thrush. I took a look at it in the mirror last night and was surprised by what I saw.

My whole pelvis is really swollen and my vagina feels distended from all the pressure it has on it from my large bump. But more worringly is that the inside part (sorry don't know the medical term) from the entrance of the vagina to the urethra isn't smooth but is all bumpy and lumpy. Kind of looks like a squashed up caterpiller with these intermittent ridges.

I had an episiotomy when delivering my DC1. That was 20 months ago. The stitching took almost as long as the delivery (I delivered within 53 mins of getting on a bed). So, I am guessing that the cut was particularly large. I've never read my notes so don't really know for sure. I didn't have any issues recovering from it but then no-one ever looked to see if it was mending the way it should so. I had always thought an episitomy was from the entrance of the vagina to the anus and the part that I am now concerned about is the opposite direction, from the entrance to the urethtra.

Just wondering if anyone else has noticed any changes like this in their vagina in the 3rd trimester? Dr Google tells me you can get varicose veins in the vagina so perhaps it is that. My next appointment with the midwife isn't for 4 weeks. I guess the best thing to do would be to schedule a non-routine appointment but I would still need to wait at least a week, assuming there is an appointment free at the next session. It's hard to know what's normal and what's not in pregnancy.

Its sounds like like a prolapse to me. Don't panic though it is very common. Try googling to see if you spot anything similar & maybe see your gp to put your mind at rest.

Thank you. I looked at some photos (hard to do in open plan working place!) but I don't think it is as extreme as a prolapse thank goodness. I rang the hospital where I will give birth and spoke to a midwife who suggested I see my GP. So I'll do so tomorrow. Not worth the worry or self-diagnosis, although the thought of someone looking down there at this stage doesn't appeal...
