Have a UTI but won't be given treatment

Last Fri went to hosp as had very heavy discharge. I was diagnosed with bad bout of thrush and just had a UTI confirmed.

I got a pessary and cream but on phone today doc said they won't give me treatment for UTI unless I get pain etc. I'm 20 wks pregnant and a bit worried.

Can this get worse and won't it be bad if I don't get treatment? Anyone know?

If it was a dipstick test in urine, sometimes some things may be positive on there which could indicate a uti. But if there are no symptoms of a uti they don't usually treat.

To confirm a uti, a sample is sent to a lab to test / grow cultures.
A dipstick may be positive for some things but it doesn't mean to say there is an infection.

If have symptoms of an uti and a dipstick is positive they sometimes start treatment as a precaution. But then to confirm an infection like I said above a sample needs to be sent to a lab for culture test.

All proven UTIs should be treated in pregnant women whether symptomatic or not. NHS use the SIGN guidelines, updated this year, which clearly state this, link here, page 16-18.
Choice of antibiotic varies and should be dependent on your stage of pregnancy and sensitivities on testing the urine sample. Usually nitrofurantoin in 1st and 2nd trimester as it has the best safety profile and is effective, or amoxicillin or cephalexin, link here.
Reasonable to repeat sample if contamination suspected and no symptoms.
If you think your gp may be difficult print these bits off and take with you. You should be treated unless there is a very good reason why not.
Hope you get a good response from your gp.

You really should get another opinion I really don't want you to freak out but one of my friends is 34 weeks pregnant and has an uti and that has led her to have contractions they have treated it and her contractions seemed to be ok at the moment but imagine if they had left her. I certainly wouldn't be happy about it.

On the flip side of that, I was once nearly in hospital with THE worst pain with a UTI (I've had enough to know it was that). I saw one GP twice who dipped, said it was negative and threw it away the first time and then dipped, got negative but sent it off to the lab saying 'if it makes you feel better' (think she thought I was putting it on). I was feeling so bad I went back to the GP and saw my own this time who was shocked I wasn't on ABs as she said I had a nasty infection that happened to only show up in the lab. SO.....if anyone ever gets a negative dip test but is sure they have an infection, I suggest insisting the sample is sent off anyway!

Go to a different GP, I had a UTI with no pain at 10 wks and was given penicillin.

sundaesundae · 28/11/2012 11:37

Looneytune this is what happened with me, luckily (or unluckily!) I have had enough kidney infections that only show cultures to know I need AB's quickly when I am symptomatic.

I am fine now, one course of cefalexin and lots of rest and fluid sorted me out :)

According to clinical knowledge summaries all UTIs in pregnant women should be treated due to potential risk of complications. I was refused an appointment by the receptionist when I had a UTI and told I would have to wait for the results (five days due to BH weekend) from a urine sample I was not allowed to drop off until the following day. I phoned out of hours and was seen straight away - I said I had leukocytes in my urine and slight abdominal pain and they gave me antibiotics immediately.

Deff go and see a different dr, there is no good reason why they shouldn't treat this. I get UTIs really badly, unfortunately i rarely have symptoms until it develops into a kidney infection and that is bloody painful.
Go back, be firm.

I went to my GP yesterday -did another sample which showed traces of protein. Explained to GP that this had been going on since my 36 week check and I'd done four samples which had same result but not had any feedback re. Abs from MW.

GP chased up at my hospital to find out what the further tests had shown and was told I had UTI. I've got no symptoms except bad smelling urine.... off to get my ABs now....
