MATB1 Form

Hi ladies

I know I can't get my hands on the form until I have been pregnant for 21 weeks (so I guess this would mean as I turn 22 weeks)>

However my notes say I get it at my 24 week appointment. I am finding work very stressful and really want to get my hands on the form so I can formally tell HR ASAP - can the midwife refuse to give me it if I book at appointment at 22 weeks?

Thanks!

GD

I phoned up the unit at 21 weeks and asked if I could come and pick it up. They just asked me to take in maternity notes and filled it our when I went. Didn't even have to see a midwife.

Mine wouldn't give it to me until 24 weeks. Get your letter to employer written so that is ready then you will be ready to give it in as soon as you get the form. The form takes 1 minute to fill in once you get it if that.

My gp did one for me, which my basset then tore up....anything important he has a knack for shredding. I wrote my pregnancy letter gave to my manager, and finally gave her my matb1 the week before I went on maternity leave!
You don't have to have the form to give formal notice, but you do need to give it at some point so you get your mat pay. But your gp can do one for you if you can't get to see your midwife.

You can legally have one at 21 weeks, nobody can make you wait.

Just call your community midwife and ask for one to be left for you.

Sure you can't tell HR without the form? They just need it in writing from you,.saying you'll give them the form when you get it.

I told HR and my line manager when I was 6 weeks as I felt so rough and didn't want them to think I was taking the mickey with days off when I couldn't cope with work. They were really good about it (obviously depends on your workplace though) the MATB1 form and written notification is purely the legal part for their records and to ensure you get SMP (which you can't take until 11 weeks before the week of your due date at the earliest anyway).

My midwife did say they were changing the dates at which they gave you your form from 21 to 24, though not sure if that's written in law and could be different for different areas (as are most things NHS related). If I were you I'd tell HR in advance of getting your form as there may be other considerations such as the type of work you do, or in my office I had to fill out a form to confirm my seating position at my desk was still appropriate, a bit Hmm but actually if you have SPD etc then very relevant. Good luck! :)

You can just tell them. The matb1 is so your work can reclaim the SMP it pays you from the government.

I had a GP appointment at 22wks to get mine. Maybe you do that?
