Maternity exemption certificate

Hi,

I need to make a dentist appointment and I'd rather not pay for it I don't have to. In my last pregnancy (years ago) I was posted my maternity exemption card automatically, however this time I've not received one and have read that I have to apply for one and get my midwife to sign the application form. Is that right?

Cheers

Yes. Her or a Gp need to sign and stamp it!

Good luck with your pregnancy x

blushingmare · 28/01/2012 08:15

Yes you'd midwife or GP can do it. Mine took ages, but I think my GP said if I had treatment before receiving it I could claim it back.

BikeRunSki · 28/01/2012 08:26

Yes, mine took about a week to arrive. Don't forget to tell your dentist you are pg. I assumed mine knew because of the exemption, but it's only the receptionist who deals with stuff like that.

Flisspaps · 28/01/2012 08:27

Yes - the MW will give you a form if you ask for it, then it needs to be signed and stamped and posted off. Mine took a week to arrive too, and it's backdated a month after they receive the form so you can claim the costs back for treatment you get in the meantime.

goingmadtrying · 28/01/2012 08:37

my mw filled mine in at the booking appointment at 8 weeks it then took about a week to arrive, i think once you've applied you qualify for the free stuff but only once its been signed, Google it to be sure. :)

blushingmare · 28/01/2012 08:43

Wow you guys were lucky - mine took 12 weeks to arrive!

Enfyshedd · 28/01/2012 20:12

My dentist took my word for it a couple of weeks ago before I was starting to show properly. I did mean to take my maternity notes with me for proof but I forgot, but they were happy to take my word for it, asked what my due date was a couple of times, and got me to sign an exemption certificate.

Come to think of it though, it might have helped that I had my scan photos in my handbag...

holls2000 · 28/01/2012 22:59

My MW took my form off to do for me, so hopefully it'll arrive soon.

MissPenteuth · 12/03/2012 11:58

Sorry, hijacking this: my mw has stamped the form and filled in her mw number, but hasn't signed it Confused They won't accept that, will they? Will I have to wait till my next appt for a new form?

I've never had to produce mine! DS is nearly 1 so it's about to expire anyway, but at dentist I've never shown it just signed a box to say I've got a mat exemption card and they have never charged me. I suppose it's like if you read the back of a prescription it tells you what can happen if you lie and say you get free prescriptions, I've never shown it for collecting those but always tick the box to have them free. I'm not looking forward to having to start paying again in a few weeks!

PoppyS34wantsgoatscheese · 12/03/2012 12:51

I had to ring MW and ask her to send me form, which I then posted off myself. I did have dental treatment before the card arrived, but it was backdated to the date of the MW appt, so the dentist took that as me being covered (iyswim?) and I didn't have to pay.

Joygirl78 · 12/03/2012 13:37

At my dentist they need the maternity exemption number on the card for their records as they claim the cost back from nhs. So you do need the card

bonbonpixie · 12/03/2012 17:23

Hi all!
Wondering if such an exemption covers private dentists too or is it only NHS ones?
Sorry for the silly question.
Have an appointment booked for 4 weeks after my due date to have an old filling replaced..... Would rather not pay for that if I didn't have to.
(sorry for hijacking!)

Pootle78 · 12/03/2012 17:40

I'm with a private dentist bonbon, however, when I told him I was pregnant he automatically signed me over to nhs treatment (obviously do nhs work for children) so same dentist but still free!!!

The GP will try to book you with a midwife or a member of stuff before that but you have to ask the reception on your own account. They don't always give you all the forms sadly! Just go to your GP's and ask the receptionist. You shouldn't have to make an appointment!
