MATB1 form - should I have it back?

I sent mine off to HR a couple of months ago, should they send it back to me?

I never got mine back. Don't know if you are supposed to. I also had to really pester HR to do anything useful with it.

No, you don't get it back.
So before you let your HR keep it, make sure your husband's work have all the info they need for his paternity leave.

Ah - well I am 34 weeks now and DH has sorted the paternity pay without the info I think. Will double check with him. Thank you.

What is an MATB1 form? Do work supply it?

it's a form your midwife should give you. I got mine about 23 weeks but you should really get given it earlier. It basically confirms your estimated Due date and you'll need to hand it in to your boss/hr dept no later than 15 weeks before your due date so they can sort out your maternity pay. I was late handing mine in but they were ok about it.

It's a legal form which the midwife signs after 25 weeks which you hand in to your employer so they can inform HMRC for tax purposes and to arrange your SMP. You shouldn't need to ask for it, they will ask you if you need it at the right time.

Oops DF is right about when you get it. I got mine at 25 weeks. It's actually issued "not more than 20 weeks before the expected week of confinement." Also if you are not employed you can give it to jobcentreplus to claim Maternity Allowance instead of Statutory Maternity Pay.

I thought my useless payroll had lost it the other day and the GP had agreed to do me a duplicate. Its not like they can deny you're actually pregnant :)
