haemorrhoids/bum grapes/piles - AGONY!

UGH!!!
I'm 32 weeks, and suffering badly from piles. I went to the GP 6 days ago, and was given prescription cream, as the over-the-counter creams were doing nothing to help. I was also given lactulose to "keep things moving", which has been a life-saver.

Anyway, as the cream has hydrocortisone in it, it hasnt to be used for more than 7 days. Today is the 7th day. I feel as if the pain is a little better, but they do not look any better or smaller than they did a week ago. In fact one of them is dark purple (which I believe is a sign that it is thrombosed).

I have an appointment again with the GP on Friday, but wondered if anyone has been through this, and can tell me what happens next...? I narrowly avoided surgery 5 yrs ago after having my son, but feel that I will not be so lucky this time.
Is there any way I can avoid surgery-type treatment is it has thrombosed...? If I do need surgery, will it be during pregnancy...?

Did the gp actually have a look and a ask you seriously about how you are feeling? They can just prescribe, thinking it is a minor case. It sounds like you are really suffering here. Mine were quite bad and really painful. Lukcily responded to the cream really quickly. Can't remember the name of it. something like pterodactyl, but obviously not that! Good luck. Make sure they take you seriously at next appointment.

Nope, he didnt look. I have suffered for years, and did tell him that, but I agree - I'd have been happier (in retrospect) if he'd had a look.
I'm hoping there is an alternative cream that may help...
IT's a different GP I see on Friday, and I WILL be looking for action this time! IYSWIM.

If they're in as well as outside, suppositories will be significantly more effective.

japhrimel, thanks for your reply. I tried the Germaloids suppositories but they didnt make any difference, as far as I could tell.
I'm now mainly worried about this one hard, purple thing - ugh!!

Hi, I had really bad piles with DD and DS, I was very silly and never went to see the GP. I had cs both times and still don't quite understand how it happened but after DS was born they got much worse and I also had a fissure (ouch, ouch, ouch) I was so embarrassed that I didn't say anything to the midwives when they were visiting at home. I ended up going to the out of hours doctor in the middle of the night about two weeks after DS was born (can't believe I put up with the pain so long) I was taken straight up to the medical assessment ward where the doctor finally had a look, this was when I first realised that I had a fissure and a thrombosed haemorrhoid. I was given some cream for the fissure and he cut out the thrombosed one. It wasn't at all painful compared to what I had been putting up with. The injection to numb it was the worst bit.

So the moral of the story is, get some one to have a look at yours and make sure they understand how painful it is. If they tell you that you'll have to wait to have them sorted at the hospital, I'd recommend you do what I did and turn up at the out of hours surgery (if it's at your local hospital?) or just go to A&E, that way they can't fob you off :)

p.s I was also given the cream that was mentioned above proctosedyl (? or something similar) I had another good cream too but can't remember what it was called.

I will do that if I get nowhere with the doctor tomorrow.
I didnt use the HC cream today, as I've used it for the maximum 7 days now. I have been in a lot of pain and have intense itching. I have used ice packs, which help at the time, but I obviously cant use one all the time. Thank God I work from home.

I then made the mistake of using a mirror and having a look. Ugh. I have been weeping at my desk since about 5pm, worried about whether the GP will do anything much, and equally worried that she won't do anything much.

Also, we're at the end of having building work done - he should be completely finished tomorrow - but I noticed a bit of the new flooring has been scuffed, and I'm certain it was the builder. But I dont have the energy to confront him about it, and I'm getting upset at the thought that - as usual - it's me who has to deal with these things, as if I havent got enough on my plate as it is.
Sorry, rant over, for now.

vmcd, so sorry you are suffering, I had exactly the same thing happened to me. I had a thrombosed hemmorhoids twice and had the blood clot cut out in A&E both times. I agree with missP it wasnt nearly as painful as it sounds and provided instant relief. Go get checked!!!! (I also cried the first time I saw them in the mirror too, over it now though!)

DS is two now and I still live with flare ups, have read loads, tried loads of things and recommend the following:

DONT eat very high fibre foods. Midwives and nutritionists will tell you to stuff yourself full of fibre, fruit and veggies (natural fibres) are OK, but DONT eat branflakes, Ryvita etc (manufactured fibres), they are very very rough on your delicate system (there is a book called Fibre Menace that explains this more)

For immediate relief of symptoms (but not a cure) put aloe vera gel on your bum. You can find it in the sun tan cream section, works even better if you put the bottle in the fridge first. Apply as often as you like.

Every time you go to the loo, put your feet up on a small stool so you are in a squatting position.Toddler steps work great, and dont look out of place in your bathroom! (look away now if you dont want TMI) This helps fully empty your bowels, and also means you dont need to push so hard.

Preventing things getting worse with your diet is absolutely the most important thing now. Try and identify what makes you constipated and avoid it! (TMI again!) Kidney beans/black beans are good for making your poo soft and therefore easy to pass.

Add a bit of apple cider vinegar to your food, its has lots of good stuff in it that aids digestion.

After baby is born (not allowed to while pregnant still) take Horsechestnut and Butchers broom vitamin supplements, this helps strengthen your veins, helps with varicous veins on your legs too, so you get double benefit!

There all done, sorry for the gory bits! Hope you feel better soon.

Mummyabroad, thank you, thank you for taking so much time to reply thoroughly. I saw the gp today who had a good prod around - yowch - and said, "oooooft!". Which was nice.
Anyhoo, she said if she referred me to have it/them removed, it would take a few days. So she gave me more HC cream, lactulose and suppositories to use at night, and said to go to A&E if I end up in as much pain as I did yesterday again.
I suffered terribly after having DS1, and have never been 100% since. This pg is just making them hellish!
I have an appointment at 35 weeks to discuss having elective Caesarean. I just hope to God they agree. I couldn't go through what happened last time, and chances are that it would be worse this time

You're welcome, glad you are getting treatment. If it helps as a reference, I would say the difference between normal (but serious) hemorrhoid pain and a thrombosed one is that strong paracetomol/cream/icepack would get you through "normal" problems. (until the paracetomol wears off off course) If paracetomol is not making any difference then go to A&E. I know hanging around waiting is not much fun, but once seen to, the treatment is quick, a little uncomfortbale (but you are feeling uncomfortable now right?) and REALLY makes you feel much better.
