Help please with 6 yo still having accidents

DS is 6 and still having frequent accidents. He's in pull ups at night, which isn't an issue in itself, but if, for example, he's in PJs before bed watching TV (or in the morning still has it on) he will pee in his pull up, often leaking onto the couch.

With his accidents, he is frequently told to go to the toilet, but we find he pretends to pee (flushing the toilet etc) or says he doesn't need, then 10 mins later he's had an accident. Unless someone goes with him and stands over him, he won't go (makes no difference if we are at home or elsewhere).

If he does have an accident, he's not bothered, he will sit there soaking wet until someone notices. He says he doesn't notice when he wets himself, or when he needs to go.

He's much better at school, I guess because he would be embarrassed if his classmates noticed (which makes me think he does know when he needs). Sticker charts and routine helps but after 3+ weeks of school holidays I feel we have gone backwards and it's been very frustrating!

Any advice please?!

Is he scared of the dark or the toilet and a bit afraid to go there alone?

Thanks, no I don't think he's scared, can't get to the bottom of it!

The school holidays you mentioned match with those in the US.

also put his nappy on right before he gets into bed, not while he's watching TV.

If it was only in his nappy I'd say it was laziness, but he's wetting his pants too.
Is he constipated?
Chronic constipation in the bladder pushing against the bowel can cause some loss of bladder control.
