Long haired cat with knots in his fur

My cat is exceptionally fluffy and not much of a groomer, he has big knots in his fur.

Whenever I see him I cut out whatever knots I can feel, but he isn't always around (bit of a tart so stays at the neighbours often).

Should I take him to the vet and have him trimmed under sedation? Or keep trying to cut the knots when I see him...

I'm worried the knots hurt him

The knots do hurt, and if you try to cut them out you may well cut his skin

I'd go ask the vet how bad they are and if they need doing under sedation.

In any case you'll need to brush him, ideally daily, to stop this happening again.

I had the same problem with my pussy (RIP Mrs Slocombe!!)
I took him to a local cattery that did cat grooming.
They did it by combing out, with no sedation.
It took all day mind you, but they got a carrier bag full of hair out, and he only had a few bald spots, it cost 35 quid and was worth every penny IMO.

I was seriously ashamed taking him, felt neglectful, but they assured me they'd seen much worse.
