Scalp psoriasis in 3yr old girl

I wondered if anyone else is/has experienced this.

My DD has thick hair and has always hated having it brushed so I bob it to the neckline.

Two weeks ago she started crying saying she bumped her head and I found a bump about tue size of an egg. When I looked it was scabby and scaly. Pharmacist said psoriasis so we got Tgel.

We went to doc the following day as she was crying it hurt so much. Doc gave us topical steroids for one week.

The bump is thick like a fried egg on her head, just above the hairline and is grimly manky. She absolutely hates having the lotion on, having it washed or brushed. We have persevered but shes so unhappy about it.

After a week of topical steroids it doesn't look much better. Baby oil has removed some of the scabbing and scales but the patch is still tuere and her hair looks awful (all greasy from the oil!).

Does anyone have any advice? My poor little girl!

Pic attached but not great as there is hair there!

Don’t worry about what it look like. Just focus on what makes it feel better.

Make sure she has a good diet and takes a multivatmin and see if you can exlimate or reduce things she finds stressful eg using visual time tables.

That is very young to develop psoriasis so I would be making a GP appointment and possibly dermatology referral for a confirmed diagnosis.

Is there someone in the family that has it?

I have psoriasis and it’s unsightly and irritating - but I wouldn’t call it painful, nor does it cause a bump the size of an egg. I’d be concerned there’s some kind of infection. Get it checked out again

We use nizerol shampoo for our children’s eczema on their scalp x it’s fabulous stuff x I would sssume it’s safe for psoriasis too but would be worth a trip back to the gp and deffo a referral to dermatology if you can x

Thanks so much.

DH has eczema and its not that. My nan had psoriasis so we assume from that.

Im going to take her back to docs this week as I don't believe the steroid lotion has made a difference and I agree with pps we should be referred to dermatology.

I wonder if it's a fungal scalp infection (https://cks.nice.org.uk/topics/fungal-skin-infection-scalp/). I had one aged about 10, a whole patch of scalp became very sore and itchy and lifted away as a crust. It was red and wept. I scratched it which of course made it worse. I used coal tar shampoo (Neutrogena T gel?) but I'm not sure they sell that any more :/

I’ve had psoriasis since I was 14, that’s 40
years, and don’t recall having lumps bar the scaly skin being a bit raised. I’d definitely see another GP and ask for a referral to a paediatric dermatology.
I found that coal tar did often calm it down, usually in shampoos or mixed in a topical cream that was often coconut oil based. you can buy / get on prescription, but I’d try with dermatology first if you can wait that long

I get scalp psoriasis when when I’m stressed and it doesn’t look like that. Not sure what it IS- but I wouldn’t jump to psoriasis.

My eldest gets something similar, and we were told it was likely a form of eczema (she doesn't have eczema otherwise, although her sister does). Dermatology prescribed Capasal shampoo, and then we have a cream that's got both mild steroid and antibiotic. She uses a mild coconut-based shampoo when not flaring up, and we put moisturiser on any patches starting to look a little pink or flaky. It does take awhile to get rid of a patch where the inflammation is well established, unfortunately.

Thank you. I will push for the referral.

Our GP had a phone call with dermatology and sent photos, much faster than going through the full waiting list, if that's an option for you.

TGel has changed and is no longer effective as they took the tar out. Get the doctor to prescribe polytar or you can just buy it. But definitely get it looked at by a doctor and documented. I have to have polytar on prescription.
