Need advice on bad nappy rash

I'm at my wits end with my daughter's nappy rash, I feel so sorry for her. She's only 4 weeks old and started getting nappy rash at 2 weeks old. It's got bad and the skin is broken and bleeding.
I'm trying everything and my whole day is revolved around the nappy free time just to get air to it.
The issue is she does not stop pooing, so I have to constantly wipe the poo away. She must poo every 30 mins- 1 hour. She's settled in herself and feeds well. I'm breast feeding and I've cut out dairy as of yesterday to see if it helps.
I've tried every cream going and only clean with cotton wool and water and pat dry. Lots of nappy free time but pampers nappies when she needs to go in nappy.
I've been to GP twice now. Tried a cream called Timodine which didn't make any difference and now they've swabbed it and given me another cream to try.
I just don't know what more I can do.
I'm also bathing her daily and patting skin dry.
My poor 3 year old is desperate to get out the house but I've not been able to as need to keep an eye on babies rash.
Has anyone been through this or have any suggestions? Please

You can buy cleaned sheeps wool in the country I live in.The Lanoline is great for nappy rash. Not sure if you can find it in uk.

Sounds awful OP. I don't have anything to add in terms of helpful comments but I've been there an it does get better.

If you had a lot of antibiotics might be worth taking probiotics - there is new advice that mum can take a large dose that will pass to baby?

When DC were little, I found using a hairdryer set to warm (not hot!) helps a lot - wash, and then make sure everything is dry before putting on nappy by using a hairdryer. Someone recommended this to me, I was struggling with bum irritation on my son...worked a treat!

No I haven't. This is my plan tomorrow when doing nappy free time later. Do I just dab it on and let it dry?

Yep, you can squirt it on direct from source.

Good luck, it's heartbreaking isn't it. Really hope you and baby get past this soon x

I haven’t read all the replies so apologies if already mentioned, but have a Google of “lactose overload”. It happens when you have an over supply and the baby doesn’t get enough of the fattier “hindmilk” (although I don’t think they call that any more). My daughter has just been through it. Two of the symptoms are very frequent, loose (and occasionally explosive) stools and nappy rash. We moved to block feeding for 24 hours (when you offer one breast per feed or one breast for a block period of time, then switch at the next feed) under the guidance of a lactation consultant and it reduced the frequency of poos in 48 hours. She was still feeding as frequently so it’s not designed to affect baby’s supply, if that makes sense. The nappy rash cleared soon after. Good luck - it is horrible.

Oh yes breast milk is always worth a try. Yes I'd just dab/splash it on and let it dry.

Shoemadlady · 08/02/2025 16:51

The best thing for nappy rash is canestan thrush cream first and they a layer of bepanthen over the top. Try changing nappies too to something that keeps wet off skin and change more often x

@Wavescrashingonthebeach Yes I did have anti biotics during Labour, every 4 hours . Labour was about 9/10 hours.

I noticed a massive difference between dc1 & 2 which i put down to the IV antibiotics. Dc1 never ever ever vomited, didn't spit up, nothing, and while they pood alot it was nothing compared to the next level back to back poo I was dealing with for DC2!!! Dc2 was always vomiting too. So I guessed that the antibiotics had given them an upset tummy. Get some probiotics x

I also did something similar to that as the hindmilk is creamier and higher in calories, midwives commented on good weight gain for a breastfed baby. Did this more with dc2. I was just kind of letting the really thin 'water' drain off a little at beginning of each feed. Think I remember looking into block feeding at some point x

The rash is no better and I just don't know what to do any more 😫 it's bleeding sores and there always poo there. It's awful and I feel helpless . Nothing seems to be working and I don't know why. I've cut dairy with no difference.

Hey @Elisabeth3468 , I was just wondering how you got on and if you found anything that helped? My 3 week old has awful nappy rash and it constantly pooping which I think is the cause 😩

Did u have iv antibiotics in labour by any chance?

laura246810 · 21/07/2025 14:38

Breastmilk is good for nappy rash. Express so its fresh, rub it in then air dry with nappy free time.

Ours was a constant pooper as well but they do grow out of it when not a newborn.

FoulWrinkledWitch · 21/07/2025 15:08

Metanium cream applied thickly worked for one of my DC who had this issue. I cut out dairy but that had no effect. Turned out DC just had very sensitive skin. Hope everything is okay now OP.

I did ! i had an EMCS so they gave me some antibiotics while i was on the operating table I think. They said they do it for everyone having a c section.

Yeah it upsets babies tummies it'll pass soon you'll get thru it xx

MudandMoet · 22/07/2025 20:34

Sorry if it’s already been mentioned but have you tried Medihoney. It’s what they use on babies with nappy rash in the neonatal ward.

YourDaringLurker · 22/07/2025 22:39

My daughter constantly pooed every time she passed foul smelling wind, it was rank mucus that stank of vinegar (sorry tmi) but it ended up being cmpa. Sort the source of the problem and I’m sure the rash will go away.

Sh291 · 23/07/2025 03:06

NHS do not recommend bathing babies every day.

DogsandDungarees · 16/12/2025 11:15

Hi , do you manage to get on top of the nappy rash ? What worked in the end I’m in the exact same position and desperate !

Meier · 16/12/2025 16:01

Hello!
her nappy rash comes and goes (it flares up if she has a cold).
the GP gave us some anti fungal cream at the time, and we just apply a ton of Vaseline at each nappy change and it has never been so bad since. Vaseline works so much better than any of the barrier creams (for us).

Yes! She's 1 now and the rash cleared up at around 7 weeks old.
I had to dab on breast milk and let it dry and then apply bepantham. It's the only thing that worked. But her poops slowed down around the same
time so I think that helped.

Thank you, I’m now doing breast milk letting it dry and a barrier cream and washing bun in sink rather than wipe. Every time I get on top of it, it comes back I am just praying she starts to poo and shart less soon! She’s 6 weeks today.
