Silent reflux or colic

i have a 4 week old baby since 3 weeks he is making a loud noises when he sleeps kind of like a scream (he seems uncomfortable and in pain)it’s not all the time and it’s definitely worse at night. I brought him to the gp who said colic I think it’s silent reflux but she was dismissive of it and said reflux is spitting up and I said I know that which is why I think silent reflux, She gave drops which havnt helped have another appointment with her tomorrow

it happens after a feed and we keep him upright for 30-60 minutes after each feed. I am breast feeding but have given him a bottle a couple of evenings before bed and it happens on both

he sounds like he is in pain red face squirming but sleeps through it. As soon as he is picked up he stops and still sleeps he stops making them if I put him in the car seat or bouncer so I don’t think it’s just him wanting to be held

does anyone have any advice I don’t want him in pain and can’t keep him upright all the time

It could be a touch of silent reflux, but if he's gaining weight well, happy to feed, and the pain isn't bad enough to wake him up, I think most medical staff are going to be reluctant to prescribe medication for him.

If you're using formula for his bottle feeds, you could try the anti-reflux or 'comfort' version of his formula and see if that helps, or the GP might prescribe carobel – it's a thickener a bit like cornflour, and can be used for both bottle and BF infants, although it's more of a faff for BF. You might be able to get it from some chemists as well. It's usually the first thing tried with reflux, before going on to anything stronger.
