Sore bum

Hi all,
New mum here, baby girl is 2 weeks old.
Has a sore bum, its red and lumpy, I feel so awful :( been using sudocrem and bepanthen, bathing with warm water and cotton wool.
I know I need to be changing her nappy asap every time she goes but it's difficult because it seems like she constantly has small amounts of runny poo every time I look. I've been on antibiotics this week for infected episiotomy wound so I'm wondering if that's affected her tum and stools...
The only thing that settles her to sleep is a feed (EBF) but then I feel I have to change her before I put her down incase her rash gets worse. But she then gets unsettled and cries and around in circles we go.
Just not sure how to manage looking after this nappy rash when she is constantly having runny poo, I'd have to be changing her nappy every 5 mins and she gets so distressed...
Any advice welcome 🙏

Congrats on your new baby, sorry to hear you both are having a rough time.
Antibiotics is probably playing a part, make sure to wipe with cotton wool and water.
Are you sure it is not thrush triggered by antibiotics? I would get her seen by a medic as she's so young.
Good probiotics once your done your course of antibiotics should help but I would get her seen by a medical professional. Also make sure to get both types of metanium creams, yellow and blue. Yellow is good for nappy rash and blue for everyday use. Hope you both feel better soon.

Second the metanium creams - we’ve had nappy rash with teething and one application of the yellow calms it right down.
Good tip for the yellow one is to apply it with a wipe (as it’s a bit of a pain to get of your own hands).

Hi I thought one of my kids had a nappy rash that wouldn't go, but it turned out to be thrush I think (it was over a decade ago so I'm a bit hazy on the details). I went to the pharmacy and they gave me a canesten cream for it and that sorted it out.

Congratulations on your newborn.

Are you supposed to continue to bf whilst taking antibiotics? I thought advice was to wait a few hours or stop altogether (not 100%) it’s been awhile since I had DD. I would run it by your GP personally as your baby is so young.

Metanium is good and zinc and castor oil cream - loads of the latter to create a barrier are good. Letting baby go without a nappy if possible.

Make up some strong camomile tea and use it with cotton wool to clean. Put a couple of teabags in her bath-it’s like a miracle cure. You can take antibiotics when Brest feeding (don’t stop them!) but it affect her tummy. She will be fine.

I find that sudocrem is way too harsh on their delicate skin when they have a sore bum. Get some metanium and it should help to clear it up quite quickly.

Metanium is amazing and also the air, nappy free time a few times a day cjred my sons sore bum at that age, his was due to constantly pooping due to having a dairy allergy we didn't know he had then. Feels terrible cleaning a sore bum alot and feeling like it'd betting worse :( antibiotics probably played a part, they do play havoc with the gut of babies, some more than others.

Worth taking to dr if it gets very bad with broken skin etc xx

All the above plus breastmilk is often said to be helpful - express some and rub on the rash. It's got antibacterial properties.

Thank you all for your advice. I'll go out and buy some metanium today for sure. I think you're right about the sudocrem being harsh. I used some bepanthen instead and it seemed better. Glad to have gotten through the night, everything seems worse then until morning comes!

Maybe a salt bath may help with the rash

Prebiotic for yourself and the baby (there are drops for the baby)
And do change a nappy every hour - I'm afraid it is an only way

I had this too with my little one (and it recurs with jabs and heat...). Every baby needs a different fix it seems!

What worked for us: bamboo nappies from Boots; Vaseline; antibiotic cream when it got so bad it bled(!); and looooads of nappy-free time - 2h a day at one point, in blocks of 20-30 minutes. We bought puppy/incontinence pads from Amazon and did tummy time and play time on the mats every day without a nappy. Still do at least 20 mins a day now at 14 weeks. You might need the fire on (even in August!) while they are very little to keep them warm enough.
