To sick or not to sick

My 9 week old daughter is exclusively breastfed and over the course of a week or so she’s been sick 3 times after a big feed before bed. She’s gotten quite fussy around bedtimes since her vaccinations and so has been wanting comfort feeds to fall asleep. I understand she’s probably taking in too much which is making her vomit but if I pull her off the boob before she’s ready she cries her heart out.

Has anyone had anything similar or have any advice?

Babies sicking up is completely normal and isn't necessarily because they've had too much. It could be trapped wind.
Personally I'd let her decide for herself when she's done and just have a muslin cloth and some wipes to hand at feed times xx

I agree with the above comment, try burping her a bit longer and see if it helps?

If this continues do you have an infant feeding team in the area you can ask for advice?

Yes happened to my dd a few nights in a row too when she wanted to cluster feed in the evenings. She would get too full and then be sick and straight away just wanted to get back on the boob. After a few days I realised that if I just kept her on one side only without switching then she wouldn’t get too much, and she wasn’t sick again after that.

Under 4 months 40% of babies will have had some experience of reflux. It's completely normal and some times symptomatic

Babies that are gaining weight and are not distressed by being sick only need behavioural adjustments. I would try either sitting with her upright for at least 20 minutes, ideally 30 after a feed that commonly triggers her to be sick and if that doesn't help try not moving her at all for 20-30 minutes. My daughter was sick the moment I moved her upright so I found not moving her more helpful.

It might also be worth getting a tongue tie assessment as they can cause babies to take in more air which triggers a bit of reflux. Another cause of reflux can be a fast letdown and leaning back so baby has a bit more control over the pace of the feed or doing lying down feeds can help

If weight gain isn't great and you find they are upset by the sick it might be worth discussing using a thickener such as carobel or gaviscon with your GP or hv for feeds that they are likely to bring up.

If things get worse - poor weight, quite to very unhappy the use of omperazole or exploring cmpa might be beneficial. I wouldn't jump to medical interventions. Start with looking at the physical options - position after feeding and getting a feeding assessment looking for tongue tie and fast letdown. A lactation consultant or tongue tie specialist would be preferable to HV and midwives and there tongue ties can be easily missed without keen eyes
