Milk allergy or intolerance?

I posted recently about severe nappy rash and doctors in a&e have came to the conclusion of milk intolerance or allergy.
My son is now 20 days old, his symptoms have been there since birth but were being put down to "normal baby behaviours while his system was settling in" and reached breaking point at day 15 with severe nappy rash and severe diarrhea. While taking his bottle it was going from one end straight out the other (aptimel)
So was 2 trips to a&e and prescribed pepti 1. I am now day 4 on pepti 1 and have noticed his symptoms going back to the way they were.

His nappy rash is clearing due to metanium however it still isn't going completely away around his bum entrance.
I've had to give him lactulose because day 1-2 of pepti 1 he ended up constipated (due to changing feeds) and was constantly straining and his stomach brick hard but wanting to feed.
I had to give him 2.5ml of lactulose yesterday.
He has had occasional spit ups but mostly it's his back end.
Now during certain feeds (he's on 12 bottles a day) I have noticed that if the lactulose hasn't worked its way, he is straining to poo while feeding. If the lactulose is working, he is still pooping while feeding. And he is constantly farting while feeding or not.
These symptoms were all the same as before, however I feel because he is now constipated, the amount of diarrhoea in his nappy isn't as much, the consistency isn't as watery either however it is reminent of the way they were on day 15 but slightly drier and not as pungent smell but still the same "type" of smell if that makes sense aka acidic?

I've been given the run around by my gp surgery, one doctor at a&e, midwifes and my health visitor. The only person who helped was the second doctor at a&e.

I myself am getting fed up of this because I can't even begin getting him into a routine. I know a lot of parents struggle with this in general but you can tell he is like clockwork with his patterns however when his symptoms worsen you are basically 3-4 nappies every change, his farting and squirming for the toilet is waking him up a lot. Thus he is then wanting part a bottle or drip fed over the space of an hour then wanting a full bottle half an hour after that because that's his "clockwork" time.
I'm beginning to lose the will to live, heh, so can only imagine how he feels when he's the one with the sore stomach.

I have now said to the health visitor that I think it's best to speak to a dietician. I don't want him suffering any longer so they can keep him on pepti 1 for weeks to see if he worsens or gets better. He is pretty much instant with his symptoms.
The way I see it is 0-7 days his gut was getting over being born and things from me. 7-14 days his symptoms progressed until day 14 they were horrific, day 15 & 16 was a&e. During the past 4 days onto day 20 I noticed last night and this morning it going back to the pooping while feeding like on day 13-14.
I feel they will argue the lactulose has mucked it up, because it's softened his poo so he can go. So it'll be a debate over that or actual diarrhoea because he's still reacting to now the pepti 1.
Something just isn't sitting right with me, the same feeling I had when I took him to a&e and they put him on pepti 1.

I don't know if extreme but I want him on the milk that doesn't have anything in it at all until his system has settled, then once it has for a while try weaning him back to pepti 1 then I will be able to see what the script is. But because he's had no "normality" I have no idea now what he's meant to be like.
They are wanting to keep him on pepti 1 to see if his symptoms get better or worse. But it's the timeline they see as sufficient which is 4 weeks whereas like I explained above he is very quick at reacting and he isn't getting better, I'm not just being paranoid.

Has anyone else went through this?
If so, what was your plan & result because I'm honestly at a loss. I keep reading people going through this when their babies are at least over a month old but not this early.
