Could this be CMPA?

Anyone with experience think this could be CMPA… all help gladly received.

DC is 3 weeks old and exclusively formula fed due to my medical need.

They have never had anything but explosive liquid poo since they were born. TMI: It’s the expected mustard colour but no where near the “paste or peanut butter”consistency.
At best, maybe once or twice it’s been a slightly thickened gravy but most often a thin liquid as the nappy soaks it up quickly.

They struggle greatly with winding and wind in general and spend hours squirming; I do all I can to attempt to relieve this and often get no where.

Over the last week they have developed what I keep calling baby acne, small red and white headed spots on cheeks, forehead and chin.
They are gaining decent weight and have no issue feeding (4oz x 6 feeds)

My only experience of CMPA is significant allergies in babies presenting with mucusy/blood streaked poo and a red facial and trunk rash.
All my other DC have had reflux ranging in severity from mild to extreme but none have had wind and nappies to the level of this one.
Usual HV is away and the stand in has been less than helpful. I feel like I’m clutching at straws with CMPA.

My youngest had/
has severe cmpa and she had what i thought was baby acne.
Her big symptom was refusing a bottle, refusing BF (like trying to breast feed a cat is the way i described it to the HV) constant scream crying and severe crying if i did try to feed her.
Could it be a milk intolerance instead?

Thank you, good to know it could be a symptomatic rash rather than acne. I think I’ll make an appointment with our GP.

Sounds like it. The rash in particular would be enough to make me think allergy. Is there any mucus in the poo? I would see the GP regardless.

Mine had a milk intolerance so it was more being in pain and squirming constantly along with constant wind. Definitely get him checked over and see if doctor will prescribe dairy free formula. Mine had soya and the difference was amazing

My 16 week old has only just got onto milk for intolerance and had similar symptoms to what you describe. I would definitely make an appointment as soon as you can! I wish we had found out about CMPA/I a lot earlier as his rash and the squirming/wind have all improved dramatically since swapping formulas and could have saved a lot of pain and tears (from everyone!). @snowyowl22 no mucus, which is what makes me doubt myself and my non-medical based diagnosis!
As gross as this is, my old DC have all had reflux and have had thicker poo which is what is making me suspicious that’s there’s something not right, I think at 3 weeks it shouldn’t be completely liquid!

@Spareincoming my son was gaining weight really well so the docs weren't thay bothered to begin with. I had to make quite a fuss to get them to listen to me! I hope your GPs are useful for you!

We had similar issues and presentation but it was due to fast let down/ over supply and tongue tie. I'd ask to be referred to your infant feeding coordinator and let them guide you.

It took ages for me to realise how bad my daughters nappies were and the GP just asked if they were normal. Ftm and lockdown they were the only nappies I had seen so the were normal to me.
We did cloth nappies too which are bomb proof and the poo never ever leaked so I thought we were better than all my friends complaining of poonamis. In disposal the poo was purely liquid and moved. As a newborn she has 10-20 poos a day and if she did one on the changing mat it fired 2 ft off the end of the mat

@Lavender14 none of those things are at play here; no tongue tie and bottle fed on slow teats, my previous DCs have all had reflux from mild to extreme and there’s no infant feeding advisor where we are - we’ve been through a lot of medical personnel over the years and often known more than them!

@LunaBear26 If I can get an appointment, the GP usually listens and takes me seriously. Mainly due to the previous reflux DC!

@Superscientist The thing is, I know they’re not normal nappies and I think I’d been hoping they’d improve but we’re three weeks in and no change.

It took me 18 weeks to get to your position!
Speak to your Dr about a extensively hydrolysed formula x

@Superscientist its only because I’ve got other DC that I’m cottoning on early! Otherwise I’d have thought it fairly normal.
I think our trust prescribe neocate which I know is a pain to make so I’ll have to research if the SMA lactose free milk is ok. I only know about SMA as it is the one mentioned in the feeding handbook of one of the DC’s childcare provision.

Neocate isn't a pain to make op. We used a perfect prep machine, absolutely fine. Or a couple oz boiling water from the kettle and topped up with cooled boiled water, if you prefer! And yes, it sounds like cmpa to me. Lactose free milk will not help as it still contains milk proteins.

Lactose free milk won’t help a baby with CMPA as they are allergic to the proteins
in the milk not the sugar which is the lactose .

Reflux and the rash can also be symptoms of CMPA. Some babies only have 1-2 and never have blood/mucus. GP will will most likely offer you a hydrolysed formula as per the guidelines (usually make you try 2 hydrolysed ) before offering an AA formula . We had a full on disagreement with our GP over getting Neocate to which our feeding nurse wrote them a stern letter . We tried 5 different formulas before Neocate made all his symptoms go and his reflux improved immensely .

@OdeToBarney and @ScoobyBooby Thank you for the reassurance around neocate - I wonder why the childcare recommend/ask for SMA; I’ve definitely had the staff tell me neocate was a pain when I was discussing DC’s reflux needs, they said they preferred the rigmarole of omeprazole tablets to making up neocate!

Here’s to hoping the Dr will listen - both the Drs are pretty good to be fair.

I think SMA is mentioned more as some children can react to the coconut oil in the Neocate .

Ive never had an issue with Neocate we use the Neocate Syneo which needs to be made at room temp which was less hassle for me preparing bottles he was 6 weeks old when he got the Neocate so took to it dead well

@Spareincoming I have no idea, that's really odd. Although if you need omeprazole again, pester your Dr for the liquid form. Mine initially tried to tell me they couldn't do it, which was a lie. They just didn't want to because it's so expensive. But we needed to be able to dose dd accurately and she was projectile vomiting the tablets! It had to signed off by our local health board, but they did it with relatively little fuss in the end.

@OdeToBarney thats very frustrating as we’ve had the liquid for all the DC with no arguments at all. I’m sorry you had to battle.

@ScoobyBooby the coconut might be the answer; the word “nut” on anything has them greetin!
