Toddler Withholding

Our nearly 4 year old is struggling withholding poop and we're looking for some advice.

The back story.. he's been fully potty trained for around a year which came naturally for him. He sometimes does rabbit type poo which we put it down to him being a fussy eater and not having a great diet. Our first mistake is we never gave him something to soften the poo like lactulose but did up his fibre which helped.

Anyways about a few weeks ago he refused to poo on the toilet which seemed out of nowhere. We put it down initially to constipation and gave him lactulose and laxido after seeing our GP. After 7 days he did a poo and then went 5 days before he went next and that was in a nappy because he was refused going on a toilet- even for a wee and was in agony. We were desperate.

The thing is his poo is not constipated anymore and he said the poo didn't hurt when he finally went but he's still holding it in. He says the poo hurts when he's fighting the urge. He'd now been prescribed picosulfate but has anything else had this issue with their child withholding? Any advice to help us through it? Thanks
