7 month old sleep - help

I'm sorry for starting what must be the millionth thread on this subject but I really could do with some advice.
My DD is just about to turn 7 months old and we can't get a grip of her sleep at all. My DS is 2.5 and has always been a great sleeper. He's got a great day nap routine (sleeps 1-3pm) and goes to bed at 7:30 with no fuss sleeping in until 7 or 730.
My DD struggles to nap in the day, if she does it is usually for no longer than 35 minutes before she wakes and can't self settle. She really fights them and doesn't show the same tired cues that my DS does (rubbing eyes, yawning, getting restless etc).
She gets tired in the evenings around 7pm and we put her down. She has her milk and goes to sleep no problem. That's when it all goes to pot.

Up until last week she'd at least slept from 7pmish to 11:30/midnight when we'd feed her and then she'd wake 2 or 3 more times between midnight and 7am for more food. She is having decent amounts at each waking (at least 50ml each time) so we haven't tried to stop these feeds because we feel she needs them?
For the last couple of weeks though she has been waking 2 or 3 times between 7pm and midnight, meaning that in total over the course of the whole night she's waking at least 5 times. We don't understand what we're doing wrong.
We've checked if the room is warm enough, have tried to encourage better naps during the day but she doesn't seem interested (even though she's clearly tired), have tried to make sure she's eating enough during the day.
Weaning isn't going brilliantly, she rejects the spoon so having to do baby led weaning so she's not consuming a lot.

Probably important to note that she had heart surgery at two weeks old so did lose a lot of weight but has eaten very well since so is gaining weight and jumping up the centiles well. We sometimes wonder if her constant feeding throughout the night is because she's still catching up to where she should be weight wise.

I just don't know how to improve the sleep situation. Any advice would be greatly welcomed.
