Nappy Rash - Nursery - Advice

looking for some advice please…

my little boy has recently started nursery and is getting really bad nappy rash. We’re trying to work out what it could be but the following clearly doesn’t help:

  • nappy will be changed less - they’re never going to change as often as us I get that
  • food - nursery food is a lot more tomato based so could be the acid
  • water - he has a sippy cup all day at home, clearly he won’t at nursery
  • teething - we all know this doesn’t help

I’ve sent in nappies, cream & Waterwipes, but even then it’s still bad.

He goes 3 days per week, and for the other 4 days we have to recover him but then it starts again. We’ve asked them to check and change more often but it’s still recurring.

any other ideas?

also I’ve been putting in the bath when I pick him up, and give him nappy free time. Would you put cream on when he’s mooching around with no nappy so just leave his bum to get air?

Which nappy cream do you use? We found Metanium (yellow tube) the only one that worked well.

Use Metanium. It is a miracle cream x

I'm going to go against the grain but metanium doesn't work for my 2.
DD has sudacrem and DS bepanthen.

Whichever cream works for your DS then use that but every other day use just Vaseline. Seems to work really well and it's what a HV advised for my DS who gets really bad nappy rash

We had the same with our boy. We frequently asked them to apply cream with every change, rather than when his bottom appeared red (because by then it was too late). He went for 3 days, we spent the next 4 recovering his skin. One day he came back and his bottom was so sore he couldn't sit down in the bath. So I wrote a formal but polite complaint. They apologised, came up with a plan which included us sending in additional cream to be kept in each changing area, they put signs up in each area reminding staff etc. No problems since then. He's loved the nursery other than that, and I really didn't want to take him out, but I definitely needed to escalate the issue.

I know it’s crazy but I get really bad anxiety when he’s not well, and nappy rash really gets to me and it’s nothing in grand scheme of things 🙈

we use Metanium and sometimes sudacreme but I use metanium every nappy too - I’ll make sure they’re doing that too I’ve bloody sent enough in.

I’ll give Vaseline a go too, good shout to create a barrier.

Hiya, we had this issue too.
I made up my own nappy cream and it's the best thing I've ever ever used on his skin;
1/3 sudocreme, 1/3 vaseline and 1/3 orabase (you can get it in boots - it's a protective paste)
Lather it on and it will stay on like a paste and completely protect the skin and heal it.

Honestly, I should copyright the mixture! It's that good

Have you tried cavillon? Can get it on Amazon, it was the only thing that cleared up my baby's stubborn nappy rash!

Is it clearing 100% during their time with you? If not there may be an added fungal element that needs treating.

We also had this issue and what helped was thin skim metanium (should be able to see redness/skin through or it's too thick) then layer on sudocrem over to form protective barrier otherwise just gets wiped off on nappy. She usually did her big poo first thing in morning after breakfast toast so if had a thick layer of sudacrem on after morning nappy change that helped prevent the worst of it.

Also any intolerances? CMPI here and we said also presumed soy intolerant too, but they were giving soy alternatives... Nappies calmed down loads since limited soy at nursery!

My daughter got terrible rash (bleeding) when she was teething, the only thing that helped was a thick layer of metanium and then a thick layer of sudocream over the top. I know that goes against the advice on how to apply but it worked really well for her.

thanks everyone will give the suggestions a go. I think I definitely need nursery to put a barrier cream or Vaseline on once this heals i‘ll get that sorted. Yeah it normally does heal when he’s with us.

I think he could have a mild milk intolerance too which has been ongoing since birth but we found Hipp organic formula has been ok for him. To be honest I haven’t even weaned off formula yet and he’s nearly 13 months, I’d rather just get rid of his afternoon bottle which I think he’s starting to do anyway. Plus he’ll be going into bigger room at nursery soon because he’s walking loads and there are no bottles in there so the temptation won’t be there like it is in this baby room.

It’s never ending isn’t it!! Always something!

I would arrange a time to discuss with the nursery outside of the usual pick up times to find out how they are checking for wet nappies etc.

Nurseries often have nappy changing routines where they routinely change/check all the children at certain times of the day, but this might not be enough for your little one or they might not need a change at that time but would immediately after for example.

They should be responsive to your concerns given that this is about your DC's comfort and wellbeing.

The yellow metanium is good for any flare ups and you can send that in as well.

My DD does get nappy rash quite bad and like yourself it does seem to get worse at nursery- changing wise they have a changing rota and change them every 2 hours (sooner if had a poo) so I know it's not that and I also send in her cream so know it's being used. The only difference I can think from home is that I make sure she is completely dry before putting on cream and new nappy- I'll often leave her 5/10 mins before putting on new nappy. I only knew this was something you were meant to do as gp told me about it when I took her about something else and she noticed it xx
