10 yr old with severe 'bottom' pain

My 10 year old daughter has bouts of severe pain that radiates from her bottom to her vagina. This has been ongoing for some time and everytime I take her to the doctors everything presents as normal. Pain has been been so severe on times I've had to take her to A & E. We have been advised not to use any soap and she only wears cotton underwear. Does anyone else have any experience of this? Pain is usually at night and can last for anything up to two weeks.

Have you ever treated her for threadworms? I know it sounds unlikely that they would cause such bad pain but my daughter went through something similar years ago. The gp suggested we treat her for worms and she was fine two days later.

We went through this. At night, crying with pain and really distressed.

It was threadworms.

Surprised Dr hasn't suggested it but as other have said threadwomrs can cause severe pain like this & usually at night. You can get OTC treatment from pharmacy so definitely worth treating, if nothing else it will rule it out.

My daughter was diagnosed with Lichen Schlerosus at five years old.m It's pretty rare in pre teens. She used to cry out at night. If it is this though you would expect to see some discoloration down below - a lack of colour in effect.

My daughter (5) used to scream in pain at night time in her bottom and genital area. Also threadworms, got some ovex and gone within a few days.

Is there any cycle/pattern to the pain- like monthly? If not worms, I wonder if it might be hormones/endo or other gynae cause?

I was coming on to say threadworms too. Caused DS terrible distress.

She has been examined by the doctor and they have never suggested threadworms. She has an appointment with the specialist soon and they are looking at two possible routes, one is gynae and the other is anxiety. The episodes usually occur when she gets excited, apprehensive, nervous. This could be birthdays, Christmas, holidays etc. Could her anxiety be manifesting as pain but I'm amazed how severe it can get. Last Sept she was admitted to hospital and had to be given Fentanyl to help with the pain.

Have you actually treated for worms OP? I'd suggest that just to rule it out.

I wonder if it’s her hormones starting to kick in, and it’s anything do with proctalgia fugax.

My DD5 has complained of a sore vulva and this has made me check immediately for threadworm. None there, thank god. Sometimes, MN really is a gift that gives.

Do you know how fast those things can shift as soon as there's a hint of light? If you ever suspect threadworms, just treat for them. Mumsnet is indeed the gift on this subject. It's all torches in the dead of night. Just.treat.them anyway.

I had the same symptoms ... no prizes.. it was threadworms. The little bustards crawl from anis to vagina and lay their nasty little eggs as they go.

They are horrid OP. Get her some ovex and hot wash all her sheets just in case. The ovex will do no harm if its not worms.

Whatever it is, it sounds miserable I hope they get to the ( ahem) bottom of it quickly.

PS Make sure her bottom is showered throughly every morning and her knickers are either hot washed or binned for about 2 weeks.

Is there shy redness? Or swelling?
My DS has had 4 episodes of something similar over the last few years (two we took him to A&E). All were diagnosed as peri-anal abscess. I do wonder though if it’s because the first time it was diagnosed as this so it just because the ongoing diagnosis as was no visible access. The first time the Dr said he could feel it below the skin (like a large hard area). His whole buttock and perineum area was very red and quite swollen. He was prescribed antibiotics all but the last last occasion in which we didn’t take him and it ended up going away after a few days (of excruciating pain and no sleep)

It sounds more like proctalgia fugax as a pp said especially if it comes on at night. It’s one of the most intense pains I’ve ever felt, comparable to childbirth. For me it was caused by endometriosis in my ‘pouch of Douglas’ (a little pocket behind the bowel). After my most recent surgery, just over five years ago, I never had it again.

I'd say threadworms too, my daughter had terrible pain to the point we saw a paediatrician. He suspected worms, he prescribed Ovex 5ml everyday over three days, two week break and then three days again. Pain went away, for the sake of one tablet I'd treat her anyway, you can get it from any chemist.

I had similar as a child and it was threadworms. Always worse at night.

I’m really surprised Dr hasn’t suggested worms. Treat the whole family - you can buy it over the counter cheaply. At the same time make sure you put fresh bedding on all beds and wash towels.

I agree with everyone else, get a treatment for threadworms from the chemist. The worst it can do is nothing.

I'll definitely be getting Ovex as soon as possible, even if it's not threadworms it won't hurt to treat us all.

I was coming on to suggest this. It feels like an agonising cramp up your rectum and around. Often happens in the night. It's an awful pain. Salbutamol or sitting on a tennis ball can help
