Nappy rash AGAIN! :(

When I had my first born she ended up getting awful contact nappy rash around her bum hole, she was only a couple of weeks old. It was cracked, bleeding and so sore!! Took a good few weeks for it to clear.

I’ve just had my second baby a week ago and I was determined to make sure he didn’t get nappy rash! So I used cotton wool balls and water, made sure he was always thoroughly dry before putting nappy on, giving nappy free time at every change and a thin layer of Vaseline to act as a barrier. But now he’s got the exact same rash that my daughter had! I’m at my wits end with it and feel really pissed off!
his bum hole area is red raw. I’m applying a thin layer of yellow metanium and bepanthan on top but it’s just not getting any better!

any other advice anyone can give me? I’m going to call docs tomorrow as he might need a milk steroid

it doesn’t help that he pops so much either! (EBF)

Have you tried changing brand of nappies? Both mine reacted to pampers but were fine with most the cheaper brands (winner!)

I use raw coconut oil on bum after cleansing and then nappy cream on top.

I agree with trying a different brand of nappies and maybe an alternative to the cotton wool (which will be bleached white so could irritate).

Skin needs to breathe and although nappies aren't great for this they are made to be as breathable as possible. However a layer of Vaseline and/or metanium is not breathable.

I found Bepanthen best for DD but used the thinnest layer possible. I have eczema and if ever it's inflamed the worst thing I can do is slather it in thick cream as it kind of makes it sweat and go raw and sore.

Something that is also important is to ensure skin is dry before applying cream. I tore some older boring muzzies into multiple small squares and used these to dab DD dry before applying cream.

A equal weight mix of yellow metanium, Vaseline and orabase was recommended for my DS’s nappy rash and it was incredible how quickly it healed it. We’d tried so many different types of cream and then steroid cream but it kept getting worse and the our Dr rang the continence nurse at the hospital who recommended it.

It went from red raw and bleeding to healed, it was wonderful.

Is there any chance he has allergies? My friend’s baby had the same issue and the paed diagnosed her with lactose intolerance..it was making the baby’s Poo acidic and causing an awful rash down there..

Ditch the Vaseline

pure organic coconut oil (if it’s hard then rub it between your own fingers for a few seconds until it melts then apply to baby’s bottom)

I don’t think it’s the nappy as everywhere else around nappy area is fine it’s literally just the bum hole area that doesn’t have contact with nappy as the hun cheeks keep them closed if that makes sense?

@mishmased I’ve not tried coconut oil but I’ve heard using it to wipe nappy area really helps so I might give this a go

@ItsNotReallyChaos I know what you mean I’m the same as I also suffer from eczema and adding product/creams only ever makes mine worse. I’m just worried if I don’t put treatment on his bum it’ll make it worse but maybe I can try it for a few nappy changes

@SpinMeRightRoundBabyRightRound did you get the orabase prescribed or could you buy it?

Soya is a common allergy too, which might cause similar reactions. I would also second allergies as causing this too. Sudocrem was always my go to for allergy nappy rash - anything with zinc oxide will help. Hope your little one feels better soon.

Might be worth trying canesten on it - my son had what looked like nappy rash that didn’t clear up and the health visitor said it was thrush. I googled nappy thrush and didn’t believe her because it looked nothing like his rash, but tried the cream in desperation and it was gone in 48 hours.

Try ilex paste. It’s absolutely brilliant stuff. Then a thin layer of Vaseline over the top. It just allows the nasty rash to fry out and clear up. Hope you’re ok, it’s a horrible thing I know.

It was prescribed the first time @SassyPants87 but then we just bought it from the chemist when we used that first lot up as I used that mix any time it started to get red in future.

We tried all sorts and I spent a fortune on fancy nappies that made no difference, my DS’s were just really susceptible to it.

My son had similar from doing tiny little squity poos that would irritate him bum but not show in a nappy etc.
I had to keep him nappy off a lot and constantly check for poo. Thin layer of metanium. I personally found bepathan on top too didn't work. Just metanium on its own & lots of air time.

@Abridget7 this is exactly what my baby does! Keeps having tiny poos that sit in his anus area that doesn’t show in nappies

how long did you do nappy free for? I can only manage 5 mins max before he does a little poo again

I will try just metanium on its own and no bepanthan on top. Doctor has prescribed canestan cream with a mild steroid to apply twice a day

Honestly it was a lot! As much as poss - atleast a few hours over the course of a day. I had a summer baby so slightly easier. I bought some cheap dark towels and I'd have him on his change mat. Even when we were out, I'd have to check for poo regularly even if nappy looked dry. It did work though, I can't remember exactly how long it took but no more than 5days to see lots of improvement if you keep on top of the squits.

I’m in the same boat at the moment with my LO. We’ve had huge bouts of really severe nappy rash all up the front and across to her legs. At 3 months we tried every nappy we could and settled on Kit and Kim after they seemed to help. Tried steroid creams, every type of barrier cream and eventually a hydrocortisone. It eventually cleared with little bouts reoccurring until she was one. We’ve now had the latest severe rash for 6 weeks, we’ve increased the hydrocortisone use with docs advice and still rotating barrier creams but this time it’s not shifted at all. So so sore, bleeding and really itching her, it’s horrendous. We’ve always used cotton wool and water during flare ups to not irritate it more and dried fully. Also done as much nappy free time but harder now she’s on the move! I’m honestly at my wits end this time and feel so so sorry for her, I can completely imagine the pain she’s in. She is distraught during nappy changes and get upset every-time she wees or soils her nappy. Watching the thread with interest as I honestly don’t know if this is going to improve until I can potty train her. I’m fed up of being told by people we need to change her more, she’s changed every 30-60 minutes, even at nursery who have been amazing with this, and as soon as she does a poo so I can 100% say it’s not us leaving her wet.. we’re going through so many nappies.
*also to note I mentioned potential intolerances to the gp and she basically shot me down!

I’ve had a really similar experience to the OP and last poster with my son, whose rash lasted several weeks as we went through many different over the counter and prescription creams. We then found one that worked amazingly (mentioned above) called Ilex. In just 48 hours the raw parts that were between his bum cheeks have almost totally healed! You can tell it’s like no other creams out there by its consistency and the fact it totally repels poo and wee, creating a total barrier that allows the damaged skin to heal.

Just to add- his rash has been on his bum cheeks and especially between the cheeks, so was constantly getting irritated when he went to the loo.

wondering how everyone is getting on? Currently have this issue with my 8 week old with what looks like blisters all over. He is distraught every nappy change and even with the nappy off. Been to GP 3x and health visitor seen it 2x. Tried multiple OTC creams and ointment as well as prescribed. Also cut out dairy this week so may be a bit early to see.

when he has nappy free time for 1+ the redness goes down so makes me think possibly nappy related?

You could try whisking white of egg (as if you were making meringue) and applying it to affected areas. It takes the heat out of the rash almost instantly and puts a fine seal on the baby's bottom.

We switched Nappies after having similar issues, moved to Kit and Kin. Whilst my LO still gets bad nappy rash when she is having teething symptoms the blisters and huge raw patches haven’t been as severe since switching. Her last bout ended up infected and we got put on Daktarin anti fungal cream which cleared it up. Ours is 100% teething related now as it dies down as soon as one of her teeth pop through.. it’s just a shame she has upto 8 weeks of symptoms before each tooth has popped out. Another poster had also mentioned mixing Orabase, Vaseline and Metanium, we made a batch of this and it was good, created a very effective barrier cream! It’s the most awful thing to have to deal with, I could cry when she’s in so much pain. Cotton wool and water for nappy changes is also the way forward as even the mildest of wipes (even water wipes) sting broken and sore skin. Hope everyone’s little bottoms improve soon 🤞🏻 X

@Sydney1986x DS’s nappy rash cleared quite quickly and these are the things I did to help

1). Applied anti fungal cream with steroid and when I didn’t use this cream I used the new sudocrem nappy rash treatment cream. It’s in a white and gold tube you can get it from boots (it’s honestly amazing)

2). lots of nappy free time but when I struggled to do nappy free I used a hair dryer on cool setting. A doctor recommended this and it really helped!

3). I used cooled boiled water and cotton wool. I also used a cream called dermol 500 to wipe his bum with as there was still friction when using just cotton wool and water. So I would dunk the cotton wool ball in water, pump some cream onto the ball and wipe his bum

4). warm baths help sooth the rash

ultimately as his gut matured and he wasn’t pooing as often this also helped too

good luck with it. It’s honestly just awful

Thanks all, such helpful advice and tips! Will get onto those. Interestingly my mum recommended egg white whisked too,
she swore by it for my siblings and I.

funnily enough re teething I think my LO is teething. I know he is only 8 weeks but he has what looks like teeth coming through on the top front two, he is dribbling, biting his hands, constantly hungry but fussy, chewing his teats, as well as the nappy rash…
