Toddler not well - any ideas?

I’m at a bit of a loss here and I’m feeling worried so just hoping anyone might be able to shed some light. It’s a long one....

Saturday, 15 month old was his usual wild self. Usually sleeps 2 hours in the afternoon but instead went down for 10/15minutes and absolutely would NOT settle back to sleep. He was grumpy and sleepy the rest of the day. Has teeth cutting through so just put it down to that. After his dinner he was practically falling asleep in his high chair which isn’t like him at all, but he usually naps and hadn’t so I didn’t think much of it. Early bath, early bed. He woke later on that night and felt very hot so checked temp and it was 39.9. Gave him some Calpol and settled him back down. Temp was down within about 30 mins so I was happy with that.

Expected him to wake yesterday with a cold etc but he was absolutely great. Bright and cheery, running around like mad, just his usual self. Temp around 36.5ish all day. Until dinner...

Had dinner as usual at 5pm ish, cleaned up and got him out of the chair and again, he’s SUPER sleepy - climbing on my knee, drifting off constantly, super drowsy and he was feeling warm again. Usually after dinner he runs around wild for an hour before bath so this was just not like him at all. I phoned 111 for advise because he was so drowsy and the temp was creeping up again. It was 37.4 when I called and after 40 mins on hold it was 38.6. They advised Calpol, put him to bed as normal as he was eating, drinking and had normal nappies, and just monitor him and they’d get a doctor to call within 6 hours.

Gave him Calpol and his usual cup of milk and he vomited it everywhere.

Doctor called a few hours later and asked me to bring him to the out of hours clinic an hour away so did that. His temp had been back down to 37.8 in the house but by the time we got to out of hours they read it as 40.1 😳 (they were using in-ear and we use digital underarm so not sure if all readings were accurate). They checked him over and gave him ibuprofen then waited half an hour to make sure it was reducing his temp. Had come down to 39.1 so they advised taking him home and alternating Calpol and ibuprofen every 3 hours overnight and just monitoring him. Temp around 38 all night, and back to 36.6 this morning. He’s running around wild, you’d never know anything had been wrong!

I’m baffled as to how he can go from being wild to being suddenly so drowsy and nothing apparently be wrong. And how at the same time each day he’s just suddenly appeared ill and temp has started creeping up. I’m now spending the day dreading dinner time incase it happens again 😓

Can anyone shed any light? If it’s an infection or something then surely his temp would be high in the day too and not just suddenly making him warm and drowsy in the evening?

Hi, it is such a worry especially with such high temperatures but I would say for mine alternating being completely fine and poorly is fairly standard. Usually completely normal in the morning but gets ill again later in the day. I've often sent him to nursery thinking he'd fully recovered (no temp, zero sign of illness) for him to get sleepy and have a temperature again around 4 or 5pm.

Thanks @Proudmamma2 😊 that’s reassuring to know!! He’s only ever been quite obviously ill before - really snotty with the cold, miserable and uncomfortable with H, F & M etc so being fine one minute then taking a sudden dip worried me a bit!
