My 2 year old badly fails the problem solving test on the health visitor checks. What does this mean?

I tried this earlier but am reposting with a better title:

DS is almost 2. He has craniosynostosis so the health visitor has been sending us the ages and stages questionnaire regularly. He is at increased risk of speech delay, ADHD and autism but I have no concerns about either speech delay or autism - he scores top marks on the ASQ for communication and 0 on the M-CHAT.

He is in the grey or white for most things on the ASQ. I am satisfied with this as he is 23 months and 2 weeks and this questionnaire is for up to 25 months However, he has always scored low on problem solving - he is a long way into the black. There are things which have been on the questionnaire for months which he still can't or won't do, such as turning a clear bottle upside down to tip out a raisin and copying when I draw a straight line on a piece of paper. He is very physical. He likes to climb things, throw things and constantly push toy cars around the house - maybe he's just not into these kinds of things at the moment, but maybe it's a sign of something he'll need further support with.

Does anyone know what the significance of a low score just on problem solving could be? What kind of developmental delay or neurological condition is it trying to screen for?
