Nappy Rash @nursery

Could I ask for some opinions. Collected DS (2) from nursery, carer said he had a nappy leak and had to have his clothes changed. The carer, not his usually carer said he had a sore bottom and had to have a lot of cream on Quite unusual for DS especially as he's close to being potty trained and will ask to be changed.

When I took him for a bath he started crying and complained of a sore leg when I had a look his leg was red raw from 'nappy rash'. He screamed in pain as I put more cream on to sooth it. I've attached some pictures. The sore is on his leg not on his bottom.
It's a really unusual place for nappy rash and 2 days later it's still red.

I've spoke to the nursery and they've apologised and had a meeting, but I'm not sure if I should take it any further.

Is it at the top of his thigh ? I’m assuming that it’s just that patch and it’s not anywhere else.
It doesn’t look like nappy rash , it looks like something has rubbed.

In fact, that’s on the outside of his nappy area. Is it a friction burn from carpet or a ride on toy ?
Can he verbalise what happened ?

That looks like chaffing like chub rub (obviously not in a child but it's the only way I can describe it and no offence to anyone btw)

That looks like where the nappy has rubbed. Either where it wasn’t put on quite properly so the kind of “frilly” edge of the nappy was caught inside which would cause it to rub, or he was wet for much longer than anyone realised so it was rubbing while he walked around. It’s not nappy rash.

I only looked at the first photo and also assumed it was inner thigh. Outer thigh is less likely to be how I described above but still possible. It does seem like he’d have had to have been left wet for a while for it to do that.

Does he wear vests with poppers ? I ask as I've seen this on children who've had a leaky nappy where the vest becomes wet and sags rubbing against their little legs. As long as the skins not broken I'd apply some Vaseline to reduce any further chaffing and whilst home let it get some air without any clothes touching it as that should speed up the healing.

Thats a rash from rubbing. Either too tight or too loose nappy. What clothes did they change him into? Is it possible the trousers were too tight?

Has the cream helped at all? I would see a nurse about it if it hasn't and if its warm enough go without trousers at home forba couple of days

We had a problem with nappy rash at nursery a few months ago. DD would come back from nursery and we'd be told she's a bit sore either nappy leaked or she had a poo that made her sore. It was often on the front and thighs and after a couple of days of stringent changing and cream at home it cleared up. Then she'd have two nursery days and it was back to square one. Nursery were making out she had some sort of rash problem.
I had to make a formal complaint that I believed they weren't changing nappies regularly enough or checking in between. Thankfully it was taken seriously and she hasn't had the problem since. So it WAS just not enough changing.

My advice is if it's a one off, just lots of cream and changing to heal the current sore area. If it happens recurrently, make a fuss.
